1. Swamper steadying the pipe being carried by the boom must always position himself
behind the load - never in front - except on steep hills where the swamper should be on
the uphill end of the pipe joint. Walking speed of the tractor should be in first gear.

3. Sling joint from the centerline mark when picking up a joint.

3. Dont stand between the pipe and the boom when rolling seams.

4. Clear your hands from the sling before signaling boom operator to lift.

5. Make sure pipe is stable on the skids before unhooking sling - chock securely at both
ends. If skids break you could be crushed between the pipe and the boom

6. Watch feet and lower leg when rolling seams. Chocks can slide out, releasing the
pipe, which then rolls onto your foot and lower leg. Make sure skid piles are secure and
not loose.

7. Don't get between pipe and tractor or pipe and bending machine - history of severe
crush injuries.

8. Never stand in a boom operator's blind spot - let him know your intentions.

9. Don't hang from a suspended pipe joint as it rises uncontrolled into the air. Don't ride
on or in the mouth of the pipe.

10. Be aware that big bends will usually flop one way or the other when pulled out of the
bender - stay clear.

11. Watch fingers when feeding pipe into the bender. Grab the top half of the pipe when
nearing the bending machine - finger amputations.

12. Don't ever get between two joints on a side hill. Use a sling hook to hook up the
sling.

13. When feeding pipe into the front of the bending machine, watch for the reach rod
coming out of the pipe - stand clear of the pipe opening.

14. Use wire rope sling, not an endless polyester sling, when roading down the R.O.W.
packing the mandrel, unless mandrel has rounded hook-up points.

15. Stand off to one side when holding the donut in the pipe - it may get squeezed out
under pressure and hit you in the chest or arm.

16. Boom hands - If you leave your boom unattended, push the safety lever down which
locks the transmission on the transmission shifter tower.

17. Mandrel Operator must maintain constant watch on mandrel control line - it may
tangle around your feet causing lower limb injury.

18. Watch toes at the hitch on the front of the bending machine - the hitch comes off the
ground then back down on every pull. Keep your feet away - crushed toes.

19. If you mount a water jug on a tractor, mount it in operator's line of sight. Avoid
mounting the water jug on bending machines danger zone - adjacent to passing
tractors/opposite side from the operator.

20. Bending machine - ensure there are steps to access operator platform. Don't use a
small stepladder.

21. Long bends in bending machine should be held up by a second boom.

22. Don't set up sections under power lines.

23. Wear hearing protection - harmful noise from tracks of booms, power units on
bending machine and mandrill.

24. Don't rush around trying to impress anybody - think before every move.

25. Always look ahead to what is coming next - stay alert.

26. Check winch line terminal-end for chafe regularly. Use correct terminal-end. Knots
can chafe and catapult the hook back toward the bender.

27. Don't walk behind, or drive a vehicle behind the bending machine while it is being
pulled up hills - hitches have broken in the past.

28. Swamper marking the pipe - keep your fingers away from the die/pipe interface. Put
the marker in a designated spot not loose on the die.

29. Unless absolutely necessary, stay out from under the fall line of the boom or its
load.

30. No walking on the pipe, ladders to be used when checking the degree of bend on
large radius bends.

31. Don't ride on the bending machine when it is being moved.

32. Watch when hooking tractor up to the bending machine - tracks on side boom are
often close to the pipe.

33. When cleaning the table or top die or cutting loose neoprene make sure everyone
knows that you are going in there.
