Due to many noted inconsistencies between the actions or inactions between General Practitioners at the
............Surgery and generally accepted scientific conclusions established over the last two hundred years or so,
I require that any GP handling my case must complete the following questionnaire in order that I may discover
current evidence of competent knowledge in the field of professional healthcare by the Crown federated agents
known as General Practitioners at the ............ Surgery please would you indicate as to whether or not you agree
