TO THE RATER: This form has been developed to monitor the performance of each intern not only for grading
purposes but also to provide basis for identifying his/her strengths and weaknesses. As a supervisor, you have a
key role in training our future hospitality practitioners.
Kindly rate the trainee in each of the traits indicated below by checking the appropriate scale that corresponds to
the objective evaluation of his/her performance in your unit/department. Also, kindly discuss the results of your

promptly as scheduled and stays productive until scheduled ending time ; routinely
takes on additional assignments.
TEAM PLAYER
The degree to which the intern displays a positive relationship within department and
with other departments.
UNIFORM / GROOMING
The degree to which the intern adheres to the personal grooming standards of the
company
ATTENDANCE
The degree to which the intern reports on scheduled days and on scheduled time.
INITIATIVE
The degree to which the intern assumes additional assignments and follows through
without being told; proactively seeks ways to anticipate and satisfy guests needs.
