REVIEW OF SYSTEMS:

No night sweats. No fatigue, malaise, lethargy. No fever

or chills. HEENT: No visual changes, eye pain, discharge.
No runny nose, epistaxis, sinus pain. No sore throat,
odynophagia. No ear pain, congestion. RESP: No cough,
wheeze, hemoptysis, or shortness of breath. CV: No chest
pains or palpitations. GI: No abdominal pain, nausea or
vomiting. No diarrhea or constipation. No hematemesis,
hematochezia or melena. GU: No urgency, dysuria,
hematuria, discharge, pain or obstructive
symptoms. MSK: No musculoskeletal pain, joint swelling.
NEURO: No syncope or seizure.
Physical Exam
Vital signs: Within normal limits.
General: Pt is alert and sitting up comfortably.
Skin: No visible bruises, scars, lacerations, or tattoos.
HEENT: Conjunctivae are white. Pupils are equal, round, and reactive
to light. No adenopathy noted. Thyroid palpable without enlargement
or masses noted. No neck pulsations visible.
Pulmonary: Breath sounds clear and equal bilaterally with no rales or
rhonchi
Musculoskeletal: No tenderness found on palpation of the chest wall.
Cardiac: No carotid bruits appreciated. Heart regular rate and rhythm.
Clear S1, clear S2; no splitting, murmurs, or rubs; no audible S3 or S4.
Abdomen: Non-tender, non-distended. Normal bowel sounds. No
masses or organomegaly.
Neurologic: Deep tendon reflexes 2+ bilaterally.
