PROCESS FOR EVALUATION AND ISSUANCE OF

PROBATIONARY SPECIAL INVESTOR’S RESIDENT VISA (SIRV)


BY THE FOREIGN SERVICE POSTS
DEPARTMENT OF FOREIGN AFFAIRS

NOTE: Prior to filing of SIRV application, the Philippine Foreign Service


Posts are requested to inform the interested SIRV applicants that only
investment in existing and new corporation/s shall be allowed as
follows:
• Publicly listed corporations
• Companies engaged in areas listed in the Investment Priorities
Plan (IPP)
• Companies engaged in the manufacturing and service sectors

1. SIRV Applicant submits two (2) sets of complete documents to DFA Foreign
Service Post:
Revised SIRV Application Form with recent photographs (6 pcs. 2X2, 6
pcs. 1x1) and Medical Certificate duly filled up and notarized. The
expiration date of the said Medical Certificate should be valid for six (6)
months from date of filing of application for SIRV.
• SIRV Form 001A for Principal
• SIRV Form 001B for Spouse/Dependents

Upon arrival in the Philippines, the SIRV holder is required to have their
Medical Certificate authenticated by the National Quarantine Office of the
Department of Health.

Authenticated Police clearance from home country. The expiration date


of said clearance should be valid for six (6) months from date of filing of
application for SIRV.

Certificate of Inward Remittance from the correspondent banks of


Accredited Depository Banks:
• Land Bank of the Philippines (LBP); OR
• Development Bank of the Philippines (DBP)

The Certificate should indicate that the amount of at least US$75,000.00


has been inwardly remitted by an SIRV applicant for placement into Peso
Time Deposit for a minimum maturity period of 30 days and a maximum
maturity period of 180 days. The certification should also indicate that
the person remitting the amount is an SIRV applicant.

Certificate of Peso Time Deposit minimum maturity period of 30 days


and maximum maturity period of 180 days. The Certificate of Time
Deposit shall have annotation that the amount cannot be withdrawn,
wholly or partially, without the prior approval of the BOI.

To be forwarded directly by LBP or DBP to the One Stop SIRV
Center, BOI within 24 hours upon receipt of inward remittance.

Accomplished Personal History Statement Form from National


Intelligence Coordinating Agency (NICA).

The accomplished NICA Form together with the authenticated police


clearance shall be forwarded by the BOI’s One Stop SIRV Center to NICA
upon receipt from the Foreign Service Posts for verification.

Authenticated SIRV Deed of Undertaking using the form provided by BOI


to DFA
• SIRV Form SH-001

Authenticated Birth Certificate/Family Registry/Household Registry

Authenticated Marriage Contract. In cases where the applicant’s home


country does not issue a marriage contract, the Chamber of Commerce
of the Applicant’s home country in the Philippines shall issue the said
certification

Original Passport

Official Receipt of payment of Filing Fee of US$500.00 for principal or


dependent/s.

2. Philippine Foreign Service Posts -


Checklist application using SIRV FL DFA 001
If complete
• Give claim stub (detach from SIRV Form 001A/1B) to SIRV applicant
• Inform SIRV applicant to pay for courier service, if he opted his/her
papers to be processed in within the prescriptive period of 20 working
days.

The prescriptive period of 20 working days starts from official acceptance of


application until issuance of visa to applicant.

3. Philippine Foreign Service Posts –


Send by courier services the original copy of application with all
supporting documents and photocopy of passport within 5 working days
upon acceptance of the application using SIRV FL DFA-001 to:

4. One Stop SIRV Center, BOI –
Evaluates application using
• SIRV Form ER-001
Within 2 working days upon receipt of complete documents from the
Philippines Foreign Service Posts
Sends copy of accomplished Personal History Statement together with
police or NBI Clearance to NICA for thorough verification of SIRV
applicant/holder.

5. If approved,

One Stop SIRV Center, BOI –


• Assigns SIRV Control Number
• Endorses approval to DFA Consular Affairs Division, Visa Section using
SIRV Form FL APL-002A
• Furnishes DBP/LBP with a certified true Xerox copy of the SIRV
applicants’ passport to be used by the Bank in establishing the
identity of the SIRV applicant.

The BOI shall retain all copies of the supporting documents.

Philippine Foreign Service Posts –


• Issue Probationary Multiple Entry SIRV valid for 6 months within 4
working days upon receipt from the DFA, Consular Affairs Division of
the authority to issue the visa
• Staples to the passport of the SIRV holder the BOI Briefing
Card/Letter requiring the SIRV holder to attend the one-on-one
briefing at BOI within 3 days from date of arrival in the Philippines;
• Forward by facsimile to the One Stop SIRV Center, BOI a certified
true copy of visa issued to SIRV applicant immediately upon issuance
of probationary SIRV;

SIRV Holder –
• Attends BOI briefing within 3 working days upon arrival in the
Philippines;
• Validates his signature of Deed of Undertaking using
SIRV Form SH-001;
• Undergo fingerprinting at the Bureau of Immigration escorted by BOI
authorized staff.

6. If disapproved –

One Stop SIRV Center, BOI –


• Informs disapproval to DFA Consular Affairs Division, Visa Section
using SIRV FL APL-003A

DFA Consular Affairs Division, Visa Section –
• Advises the Foreign Service Posts of the denial of issuance of
probationary SIRV;

Philippine Foreign Service Posts –


• Inform SIRV applicant of denial using SIRV Form APL-003

SIRV Applicant –
• Requests the One Stop SIRV Center, through the Philippine Foreign
Service Posts, for withdrawal of his/her time deposit place at the
LBP/DBP indicating the name, address and telephone number of
correspondent bank and SIRV applicant’s account number where the
FX or Pesos will be remitted.
• The foreign exchange remittance converted into pesos and placed in a
time deposit can be withdrawn only after 30 days from date of inward
remittance.

One Stop SIRV Center, BOI –


• Endorses to LBP/DBP request for withdrawal using SIRV Form APL-
003B copy furnished the Philippine Foreign Service Posts.

DBP/LBP
• Remits to designated Bank of SIRV applicant the principal amount
including interest.
• Issue certification to the One Stop SIRV Center that amount remitted
has been withdrawn and/or remitted to designated bank.
