Use of foil to block out extraction sites prior to relining immediate dentures

At the placement appointment for an immediate denture, a tissue conditioner is often used to compensate
for discrepancy in the fit, ensuring patient comfort and
stability. When the stone cast is trimmed conservatively,
or there is unanticipated need for additional surgical
trimming of the extraction sites, the denture will require
an interim resilient liner.1 However, if the liner is placed
directly over the extraction sites, it can disturb the clotting mechanism and pull out suture material when the
denture is removed after initial polymerization. Protrusions on the intaglio surface of the denture also require
trimming. This article presents a simple technique to
block out the surgical sites before relining the immediate denture.

PROCEDURE
1. Dab the extraction sites gently with sterile gauze.
2. Cut the sterile adhesive foil (Burlew Dryfoil; Jelenko,
Armonk, NY) to appropriate dimensions to cover the
surgical sites, as well as the alveolar ridge.
3. Place the foil, adhesive side down, on the residual
ridge (Fig. 1).
4. Mix the monomer and polymer of the resilient liner
(Microseal; American Consolidated Mfg Co, Conshohocken, Pa) to the manufacturers specifications.
Load the material onto the intaglio surface of the
denture and place the prosthesis intraorally.
5. Perform border-molding movements. Have the patient occlude gently with the antagonist prosthesis,
if present.
6. After polymerization of the liner, remove the denture, trim any excess, and tease the foil away from
the intaglio surface of the prosthesis with cotton
pliers (Fig. 2).
