SWORN AFFIDAVIT
I, CATHLEEN MARIE M. MANZALA, Filipino, of legal age, single and with
Office Address at 3rd Floor, PHINMA Properties Center, No. 29 EDSA, Mandaluyong
City, after being sworn in accordance with law, depose and state that;
1.

I am employed as an HR Assistant with PHINMA Property Holdings

Corporation (PPHC), a corporation duly organized and existing in
accordance with Philippine laws, with Office Address at PHINMA
Properties Center, No. 29 EDSA, Mandaluyong City.

2.

I have been duly authorized by the corporation to execute this sworn

affidavit for purposes of cancelling the following names: MARIA MAE
P. RUIZ, DONATO G. ORIJUELA, EUGENIO B. YAP and MATUTE A.
PELI JR. to the master list of those employees which have outstanding
loans with the SOCIAL SECURITY SYSTEM (SSS).

3.

In connection with the discharge of my duties and responsibilities as

such HR Assistant, I have to maintain and update personnel records for
staff and to ensure that the same are being filed on a regular basis.

4.

Our records show that the following above mentioned names are not
bona fide employees of PHINMA Property Holdings Corporation (PPHC)
and as such must be deleted to the master list of the companys
employees who have outstanding loans with the SSS.

5.

This Affidavit is being executed to attest to the truth of the foregoing

narration of facts and in order to support and facilitate the request to
delete the above mentioned names to the master list of those
employees which have outstanding loans with the SSS.
