Application for Surveillance - Associate Welding Engineer

I refer to our letter of 7 May 2014 on the above subject.
According to the Standard for Certification of Welding Co-ordination Personnel
(Associate Welding Engineer) with reference to the document JWES WES 8103:
2008 you are required to apply for the JWES surveillance of your job from the
aspects involvement in welding and related activities.
Please complete the attached application form and return it three (3) months
before the expiry of your Certificate, to the address below, together with the
application fee of RM 250.00 made payable to the Malaysian Welding & Joining
Society. You may bank into the Societys account (below), and forward the bankin receipt together with the application form to:
