Parent Permission Slip for Chapman Turkey Trot

Dear Parents,
Your child is eligible to participate in a school-sponsored activity requiring transportation to a location
away from school premises.

Student Cost:
$3.00
Parents attending the Turkey Trot will be required to pay $10.00 upon entry to the park, unless you
have an annual park pass. Winners in each grade will receive a certificate and trophy. The $3.00
admission covers the cost of a snack and drink after the run, awards, and transportation. If you would like
your child to participate in this event, please complete, sign and return the bottom half of this form with
$3 attached to school by Wednesday,

November 16th.

I hereby consent for my child,_______________________________________ to participate in the
Turkey Trot on Wednesday, November 23, 2016. I understand that this event will take place away from
the school grounds. I further consent to the conditions stated above for participation in this event,
including the method of transportation.
In consideration of my child being allowed to participate in this event, I agree to indemnify and hold
harmless Chapman School, any and all affiliated organizations, its/their employees, agents,
representatives, volunteers and/or drivers, from any and all claims I or my child may have, excluding
claims for intentional misconduct or gross negligence, arising from or relating to my childs participation
in this event.
I authorize Chapman School officials to obtain necessary medical treatment for my child in case of
illness, injury or accident. My child has the following medical conditions or allergies that a health care
provider should be aware:

Parent volunteers are also greatly needed, especially at the finish line and along the course, to help
make the Turkey Trot successful. If you are interested in volunteering, please indicate your interest on
the back of this page. We will not have use of the snack bar this year so if you have an easy-up type of
tent that you would be willing to put up at the finish line area to create a spot to pass out snack and
drinks, I would very much appreciate it!!

We need lots of parent volunteers for the Turkey Trot. Eight people are needed to work the finish line,
four or five to block turn offs on the course, and as many as possible to walk/run with the students.
