Nursing Care Plan


(Incarcerated Inguinal Hernia)
Nursing Diagnosis
Acute pain related to swelling of the right testicle as manifested by discomfort in the
groin area especially when bending over, coughing or lifting.

Nursing Inference
An incarcerated hernia can become strangulated (can stop blood flow to the tissue) which
can contribute to tissue damage, thus causing pain and swelling.

Nursing Goal
After 2-3 hours of rendering nursing intervention, the patient will be able to feel lesser
pain as will be manifested by decrease swelling and more comfort on the groin while bending
over, coughing or lifting.

Nursing Interventions:
1. Accept patients description of pain. Asses the pain by using the 10 point pain rating scale
as basis for assessing and evaluation.
2. Provide or teach comfort measures to alleviate the pain such as touch, proper breathing
exercises, music therapy and other relaxation techniques.
3. Administer and analgesics as ordered.
4. Reduce or eliminate factors that precipitate or increase the patients pain experience such
as to avoid bending over, coughing and lifting heavy objects.
5. Advise and give comfort measures for surgery.

Nursing Outcomes
After 2-3 hours of rendering nursing intervention, the patient was able to feel lesser pain
as manifested by decrease swelling and more comfort on the groin while bending over, coughing
or lifting.
