Case #1

DH is a 74 YOM presenting with increased fatigue and weakness. DH is a fairly


active 74 YO, but over the past two months has noticed increased fatigue and
inability to complete his normal daily activities. Patient states he is having regular
BM, but has noticed they are darker than usual. He denies any other complaints.
PMH:
HTN
DM
Arthritis
BPH
Medications:
Lisinopril 40 mg PO daily
Metformin 1000 mg PO BID
Tradjenta 5 mg PO daily
Tamsulosin 0.4 mg PO daily
Naproxen 220 mg PO daily PRN
Tums PRN
Allergies: Amitriptyline (hives)

BP: 155/85 mmHg
Temp: 36OC

P: 74 bpm
Ht: 72 inches

RR: 18 breaths/min
