Abstract
Background : Selective intrauterine growth restriction (sIUGR) occurs in around 1015% of

monochorionic (MC) twins. The condition is increasingly considered to be an important


complication of MC twins, with potentially significant risks for both fetuses. Unequal placental
sharing is the most important factor determining the appearance of growth discordance, but the
clinical evolution is largely influenced by the pattern of placental anastomoses. Thus, MC
pregnancies with sIUGR may be associated with remarkable differences in clinical behavior and
outcome.
Objective : To evaluate uncommon case, selective intrauterine growth restriction (sIUGR) in
monochorionic (MC) twins
Methods : Case report
Result : a 24-years old primigravida pregnant woman with twins monochorionic diamniotic,
diagnosed since 24 weeks gestational age. Patient goes to the fetomaternal polyclinic routinely.
In development, there were significant differences in body weight between the first and second
baby. US examination did not reveal any disturbance in blood flow from both of babies, there
were no signs of twintwin transfusion syndrome (TTTS) based on Quintero criteria. At 38
weeks gestation, pregnancy was terminated by cesarean section of indication selective IUGR and
AEDV on the small fetus. Both of the babies are female, with body weight 2400g and 1300g,
body length 49cm and 39cm.
Conclusion : In this case, both of the fetus has significant body weight difference. In
development of the pregnancy, intrauterine fetus growth ran well until the age of full-term
pregnancy. Although there are many risks that can happen in selective IUGR case, the condition
from both of the babies after birth is in a good state.
