WHAT SHOULD I DO WHEN?


A. In the event of accidents/injuries: Attach copies of any relevant documents.

What are the schools injury policies? (e.g. What should you do if a child gets
injured?)
First notify Monique about anything that happens
What documents must you complete?
Parental document and/or district document
Where is the first aid kit?
In the closet of the cafeteria
Who should you contact in the event of an injury?
Always contact Monique.

B. In the event of fire/evacuations: Attach copies of any relevant documents.

What are the schools fire procedures?


Evacuate to the grass in organized lines, keep count of kids.
Where are the fire exits?
Grass area is best exit
Where is the meeting point?
Front or side of school

C. In the event of unauthorized access of children: Attach copies of any relevant


documents.

What are the schools procedures should an unauthorized individual attempt to


access the children?
Never release a student on our own, we must talk to Monique first.

D. In the event of school lockdown: Attach copies of any relevant documents.

What are the schools procedures should a school lockdown be enacted?


There are portables near the grass area, or the bathrooms are available as well.

ABOUT THE ACTIVITY SPACE:


A. Risk assessment of activity space: Identify all potential hazards in the
OUTDOOR activity space (draw a map of the area and indicated all potential
hazards).
B. Risk assessment of activity space: Identify all potential hazards in the
INDOOR activity space (draw a map of the area and indicated all potential
hazards).
C. Where are the closest restrooms and water fountains?

ABOUT MY STUDENTS:
A. Risk assessment of students medical needs:
- Identify all students with specialized medical needs.
- Be aware of student profile/medication and identify school medical
personnel responsible for these students
Ask Monique for any medical files if it is necessary

PRE-PROGRAM PREPARATION:
A. Where is the equipment located and how will I access it?
All equipment is in the shed, ask Monique or a janitor for access.
B. Where will I go if it rains?
Either in the cafeteria or classroom with the ramp.
C. How will I make sure the students are adequately protected from the
weather (sun/rain/wind etc)?
If necessary we will go to the same places if it is raining
D. What will I do if a student is not appropriately dressed for Physical
Education?
We will send the child to Monique
E. Where can I leave my personal belongings?
In the classroom by the cafeteria
F. What access, if any, do I have to the teachers lounge/facilities?
We can use the Teachers lounge or the cafeteria, we must not use the
student restroom.
