05/24/12 - SNU, day 80 (including day of admission). Waiting for
transfer to St. Dominic's. Denied continuing coverage of
hospitalization effective 04/06/2012. Transfers safely to any sitting
surface with minimum assist with caregivers. Feeds self with utensil
appropriately >50% out of meal tray while sitting upright in chair with
supervision (met goal); Continuing OT tx plan for transfer training.
05/24/12 - SNU, day 20. Continuing physical rehabilitation.
05/24/12 - SNU, day 17. Continuing physical rehabilitation /
deconditioning. PT Rx 4 weeks PT.
05/24/12 - O2 sat latest = 90% by ventimask. No new notes, no new
orders.
05/24/12 - s/p CT-guided liver biopsy done yesterday, 05/23/2012 at
GRC. Patient stable. On IVNS + KCl at 80cc/hr. On regular diet.
05/24/12 - discharged today at 1243H. No surgical intervention done.
05/24/12 - facial hematoma decreasing. (+) osteoporosis, continuing
CA 500 TID. R hip pain, referred to Dr. Bollinger.
05/24/12 - scheduled for colonoscopy today, Dr. Bates.
05/24/12 - discharged to home this morning.

05/24/12 - (+) foot wound culture for MRSA, Group A&B betahemolytic strep. Rx Clindamycin IV q8H and Ciprofloxacin.
