My Child as a Learner

(Adapted from Practical Aspects of Authentic Assesments: Putting The Pieces Together; Hill and Ruptic)

Since you are your childs first and best teacher, your perceptions of your child as a
learner are very valuable to me as his/her classroom teacher. Dont feel like this is an
assignment where you have to send in your homework, but rather just use it as a guide to
reflect on your childs development and needs at this point in time. Feel free to use this
form or e-mail me your thoughts. Thank you so much! I will enjoy reading about your
child.

How does your child seem to feel about coming to kindergarten so far?

What are your main goals for your child this year?

What does your child seem especially interested in at home?

What have you identified as your childs strengths? Are there areas in which he/she may
need support?

Do you have a regular reading time together? What types of books does your child
enjoy?

What are your observations about how your child plays with others? Does she/he have
opportunities to play with friends outside of school?

What do you observe about your childs ability to focus? To listen and understand? To
follow directions?
