The following forms need to be completed and returned to the Building Department:
1. Fire Hydrant Availability w/ attached approved Water Availability form.
2. Water Availability with the Original Signature (This form will be filled out by your Water District)
3. A detailed plot plan with addresses of adjoining properties. This plot plan must be drawn to scale.

This form must be completed and signed by personnel from the appropriate agency to insure adequate water availability. The
completed form and plot map must then be returned to the City of Des Moines Building Department as part of your application
packet. This certificate provides the Engineering Department with information necessary to evaluate development proposals.
Please be aware that South King County Fire and Rescue makes no representation that this availability will be accepted by
the applicable land use authority.
