AFFIDAVIT OF DISCREPANCY

I, Cherry Grande Puazo, of legal age, married, Filipino and a resident of Brgy. Balite, San Jose,
Northern Samar, Philippines, having been duly sworn in accordance with law, hereby depose and state:

1. That I was born on August 3, 1986 to spouses Felipe F. Grande and Vilma M. Grande as
evidenced by the Certificate of Live Birth issued by the Municipal Civil Registrar of San
Jose, Province of Northern Samar, a photocopy of which is hereto attached as Annex A;

2. That I, in good faith and acting on the honest belief that I was born on August 3, 1986, had
used August 3, 1986 as my birth date in all pertinent school records as evidenced by
Transcipt of Record issued by Asia College of Advance Studies in Arts, Sciences, and
Technology, Inc. of Bobon, Northern Samar, a photocopy of which is hereto attached as
Annex B;

3. That when I filled up my voter's registration record required by the Commission on Election,
I wrote August 3, 1986 as my date of birth as evidenced by my Voters ID, a photocopy of
which is hereto attached as Annex C;

4. That however, when I applied for a Marriage Certificate by the Municipal Civil Registrar of
San Jose, the Municipal Civil Registrar Officer in charge of the application inadvertently and
erroneously wrote August 3, 1987 as my birth date instead of August 3, 1986 as evidenced
by the Certificate of Marriage issued on June 5, 2011 with Marriage License No. 4839002, a
photocopy of which is hereto attached as Annex D;

5. That said discrepancy in my Certificate of Marriage was an error done inadvertently;

6. That I am executing this Affidavit of Discrepancy to attest to the truth of the foregoing facts
and to explain the discrepancy in my Certificate of Live Birth, Transcript of Records, Voters
ID, and Certificate of Marriage as regards to my date of birth;

7. Further, I am declaring that from this date hereon I would be using my true and correct
date of birth of August 3, 1986 in all my papers, records and other documents.
