Djenkolism Toxification

Definition

Djenkolic acid (or sometimes jengkolic acid) is a sulfur-containing non-


protein amino acid naturally found in djenkol beans of the South-East
Asian legumes jengkol (Archidendron jiringa).

This compound consists of two cysteine radicals connected by a


methylene group between the sulfurs, or it is 2-amino-3-[(2-amino-3-
hydroxy-3-oxopropyl)sulfanylmethylsulfanyl] propanoic acid. It is toxic
to humans, especially causing nephrotoxicity.

Epidemiology

Djenkolism is considered an uncommon but important cause of acute


kidney injury in tropical Asia. Djenkolism sporadically results from the
ingestion of the djenkol bean (Archidendron pauciflorum) native to
Southeast Asia.

Despite its strong smell, the beans are regarded as a delicacy in some
areas, including Indonesia.

Etiology

Djenkolic acid toxification is produced when consuming djenkolate acid


present in djengkolic beans.

Symptoms

spasmodic suprapubic
flank pain
urinary obstruction
acute kidney injury
oliguria
nausea
vommiting

Diagnosis

Anamnesis on wether the patient have consumed the djenkol beans


odour of the patients urine and breathe.
Physical Examinations for signs of urinary tract obstructions
Laboratory Examinations

Classification

Light

Spasmodik pain and near the hips at the suprapubis region within 1-2
days.

Heavy

Colic Pain at the abdomen region ,nausea, diarrhea, disuria.

Treatment

Hemodialysis

Increase the flow of urine by being hydrated so that it can excrete the
djengcolic acid remains from our body

Complications

Djenkolism toxification along with nephropathy obstruction due to the


djenkolic acid crystals

Hidronefrosis due to the urinary tract obstruction.
