SUBJECT : Deferment for FBRC Training

DATE : 06 October 2016

1. Reference: Bureau Order #: TRN-2016-059 re: List of Participants to undergo the


Fire Basic Recruitment Course (FBRC) Training at the National Fire Training
Institute at Camp Vicente Lim, Calamba City, Laguna which will commence on 12
October 2016.

2. Last 12 April 2016, I undergo the operation called varicocelectomy and was advised
to rest and do light activities only. When the Bureau order was released, I first
sought the advice of my medical doctor who attended me on my operation since I
felt pain on my operation site occasionally during these past months. The doctor
advised then that I should rest for another 3 months with light activity restrictions to
fully recover.

3. ITCON, the undersigned would like to request for deferment from training.

4. Thus any favourable consideration taken regarding on my concern shall be looked-


upon with deep sense of gratitude. Thank you and God Bless Sir.

5. Attached herewith is the Medical Certificate issued by my doctor for your ready
reference.
