2017 may be admitted in the Polytechnic
University of the Philippines Senior High
School (PUPSHS) based on the following:

assessment of your online

b. Two (2) pieces of 2x2 inch. recent
picture with white background and
applicants name tag at the chest
c. Original:
Form 138 (Grade 10)
PSA Birth Certificate
NCAE
Certificate of Good Moral
Character
d. Photocopy:
Certificate of Completion
Baptismal Certificate
Grade 10 School ID
e. Place in clean, long brown envelope
with Surname, Given Name and
Middle Initial written at the upper
left hand corner of the envelope.
f. LRN (Learners Reference Number)
must be indicated in the Report card.

Voucher Recipient) is strictly
required to those coming from the
