Camp instruction will be provided by the Bellevue West football staff, collegiate
alumni, and current varsity football players.

I hereby give my son permission to attend the 2017 Youth Camp, sponsored by
Bellevue Public Schools. Furthermore, I release the Bellevue West staff and
school district from all claims, which may occur while my child is attending the
camp.
