INFORMATION

The undersigned accuses of JOHN DOE AND PETER DOE of the crime
MURDER as defined under Article 248 of the Revised Penal Code, committed as
follows:

That on or about DATE , in the City of PLACE, within the jurisdiction of

the Honorable Court , the above-named Accused, conspiring,
confederating and mutually helping each other, Accused JOHN DOE
acting as the driver while PETER DOE acting as the hitman, armed with
an unidentified caliber of firearm, with intent to kill, willfully, unlawfully
and feloniously shot MARY DID, causing her eventual death, to the
damage and prejudice of his surviving heirs.

The crime is qualified with treachery considering that the killing

sudden and unexpected, without the slightest provocation on the part of
MARY DID rendering him bereft of any means to defend himself.
