No. HSEMS:04-016-2012(PRO) Type: Procedure (PRO) T Next Rev. 2014 Page 1 HSE Management System - Check Performance Moni toring 16.0 16.1 16.2 Purpose The purpose of this procedure is to monitor, measure and report health safety and environmental social performance, such as: ‘* Significant environmental aspects and hazards activities at regional and project level. * Applicable management controls of these impacts and risks. * Measurement of management programs, objectives and targets performance. * Compliance with applicable environmental, health and safety legal and other requirements. * Report and record monitoring results to facilitate subsequent corrective and preventive action. Scope This procedure applies to all STS functions and operations throughout all contracts and locations. Terms & Definitions * Monitor / Measure ~ check, quantify and evaluate STS health safety and environmental and social performance derived from ‘exploration activities. * Reporting - report of environmental and health and safety performance * Health Surveillance - is carried out by an occupational health specialist to measure employee health in order to detect the potential for or actual adverse effects at an early stage, enabling further harm to be prevented. HSEMS:04-016-2012(PRO} Type: Procedure (PRO) ‘Next Rev. 2014 Page 2 HSE Management System — Check Performance Monitoring Measuring Performance @ Performance is to be measured on a regular basis, appropriate to the needs of specific business activities. © A report of HSE performance is to be integrated with company’s annual report, detailing the extent to which the objectives in the company performance indicators and the business location HSE plans have been met. Proactive Monitoring © Proactive monitoring is to be carried out in one or more of the following ways according to the needs of the company; ¥ Direct observation by managers of specific business activities to monitor the achievement of HSE standards and/ or objectives, ¥ Periodic inspections of HSE documents. Y Managers of specific business activities assigned to inspect premises, plant and equipment. ¥ Health surveillance, as appropriate. Objectives — state what is intended to be accomplished. Targets - define a detailed quantified performance requirement that arises from the HSE Objectives that needs to be met in order to achieve those objectives. Performance Indicators - are measures to review whether objectives have been met. They include rates, ratios or indices which reflect how well the OHS management systems or its elements are operating ‘Doc. No. HSEMS:04-016-2012(PRO) “Type: Procedure (PRO) ‘Next Rev. 2014 Page 3 HSE Management System - Check Performance Monitoring + Reactive Monitoring © Reactive monitoring is to be carried out in one or more of the following ways according to the needs of the company; ¥ Injuries and ill-health analysis, ¥ Examination of other losses (e.g. damage to property), ¥ Reporting incidents with the poter or loss, to cause injury, ill-health ¥ Identification of weaknesses or omissions in performance standards. © The Managing Director is to decide which of the measures are appropriate based on the outcome from the Operation Analysis and Control process, audit reports and feedback from within the workplace. 16.3 Performance Requirements Health, Safety, and Environmental performance data is collected, analyzed and reported to monitor and evaluate ongoing HSE performance and drive continual improvement. * Processes are in place to ensure HSE performance is regularly measured, monitored, recorded and analyzed with results are reported appropriately. * The reported data is collated and analyzed, as appropriate, and reported to personnel who have the responsibility to take action on the basis of this information. Reporting of consolidated data Including HSE key performance indicators, shall be carried out at site level, Business Unit level and for the whole of origin, as the basis for decision making. « HSE department provide HSE reporting requirements and processes and maintain a database containing what is required for both internal performance evaluation and external reports. Next Rev. 2014 Page 4 HSE Management System - Check Performance Monitoring 16.4 16.4.1 16.4.2 * Business Units measure and report HSE performance data on a monthly basis or more frequently if appropriate (e.g. for Significant Incidents). * Prevalence and incidence of work-related injury and illness is recorded, assessed and reviewed using data available from all sources including medical surveillance programmes and incident related medical treatment, * HSE department periodically aggregate and analyze HSE performance data for all businesses and provide feedback and advice to the businesses on initiatives to drive improvement in HSE performance. ‘+ Systems are established to ensure the appropriate reporting of HSE performance to relevant authorities, as required. * Relevant HSE systems and performance information is routinely communicated by management to relevant personnel. Responsibilities CEO & Top Management * Responsible for establishing a system for Performance Monitoring & Measurement + Responsible for assuring HSE performance is regularly monitored and necessary reports and communication is made to all concerned. * Ensure all necessary assistance to Dir. HSE, HSE Advisor and Operations required for performance monitoring. HSE Advisor * Advice the Top management on HSE monitoring, measurement and reporting. ‘Dos: No, HSEME:04-016-2012(PRO) ‘Type: Procedure (PRO) Next Rev. 2014 Page 5 HSE Management System — Check Performance Mo: ring enn Oe 16.4.3 * Compile and forward report to CEO & Director HSE on HSE implementation and performance. * Determines that a necessary authority has been assigned for collecting HSE Reporting. = Conduct all necessary checks, complete relevant forms and upload in the HSE System. * Provide information to the TOP Management on all activities conducted on a monthly basis. + Report to Top Management on the accomplishment of objectives, targets and management programs. Operations Managers + Ensures that all HSE monitoring criteria is well understood and comply with, ‘+ All reports must be submitted on timely basis. * All HSE deliverables must be recorded and necessary data and statistics to be communicated. 16.5 Procedure * The performance of HSE management systems is critical to the achievement of organization's policy, mission and vision. Also, additional performance indicators may be established for the organization to measure HSE performance There are two types of statistical techniques used to validate HSE performance. These include: © Proactive (positive) performance indicators; © Outcome (negative) Indicators. Proactive Performance Indicators Proactive performance indicators measure the actions performed as part of the HSE management system. Proactive performance indicators are designed to measure the actions being taken by STS to prevent work related injury and illness. Proactive performance indicators that are used at STS include * Training Performance Indicator ¥ The number for training courses delivered or coordinated. ¥ The number of hours for training courses delivered. ¥ The number of participants in training courses delivered. ‘Doe. Procedure (PRO) ‘Next Rev. 2014 Page 7 HSE Management System — Check Performance Monitoring 16.5.1.2 + Hazards & Incident Reporting Performance Indicator ¥ Number of hazards identified (unsafe acts & unsafe conditions). ¥ Number of near miss reporting. * Safe System of Work Performance Indicator ¥ Number of too! box talk. ¥ Number of PTWs ¥ Number of Isolations. ¥ Number of safety tours & Inspections ¥ Number of Risk Assessments Developed. * Communication & Commitment Performance Indicator ¥ Number of Daily safety meetings/talk ¥ Number of weekly safety meeting by management. ¥ Number of CEO & Top managements visits Outcome Indicators Outcome indicators are statistics which measure _ the successfulness of the implementation of the HSE management systems implementation in achieving the objectives as stated in the HSE Policy. Examples of outcome indicators that may be analyzed at the STS include: » Lost Time Injuries Qutcome Indicator ¥ Lost Time Injury Frequency Rate(LTIFR) A frequency detailing the amount of lost time injuries per million hours worked. ‘Dec. No, HSEMS:04-016-2012(PRO) ‘Type: Procedure (PRO) Next Rev. 2014 Page 8 HSE Management System — Check Performance Monitoring * Medical Treatment Cases Outcome Indicator Y Medical Treated Injury Frequency Rate (MTIFR) A frequency detailing the amount of medically treated injuries per million hours worked. 16.6 Reporting Process * Reporting of HSE performance indicators should be carried out on monthly basis. * Incidents of high risk rating must be informed immediately. ‘* The reporting of statistics should be carried out on a prescribed format sheet. + Incase of clarification to statistics or data entry contact HSE Advisor. 16.7 Records Following records must be kept; * All records confirming the statistics. * All records referring to incidents and accidents, * Records of Indicators, whether performance or outcome. * Statistics sheet sent on monthly basis.
