AFFIDAVIT OF DESISTANCE

at __________________, Bohol, under oath allege:

1.That I am the Father of minor offended party,

______________________ who filed a complainant in
_________________ for alleged "RAPE" against accused,
_________________, which case is now pending for resolution
before the Provincial Prosecution Office of _________;

2.That against my daughter's will and thru force,

violence and intimidation, I forced her to execute a Sworn
Statement taken by _____________________;

3.There is no truth to the allegations contained in her

sworn statement executed on ______________ at
__________________________ and also in my affidavit which I
executed on the same date. There was no force, violence or
intimidation done against me by said accused
_________________________. I was not forced by him to
submit to his desires;

4. I, therefore respectfully request that the above-

mentioned case filed against __________________, be
DISMISSED as in fact I am now withdrawing the complaint
against accused and I further manifest under oath that I
am now desisting from testifying against him in Court or in
any other government entity or agency in connection with
the said criminal case.
