1. That I am the same person who executes this Affidavit of Undertaking;
2. That I am a member-beneficiary of ________________, assigned at Block ___, Lot __; 3. That the lot assigned to me is in pursuant of the Lease Purchase Agreement (LPA) entered into between me and the said HOA; 4. That the purpose of the lot assignment is exclusively for residential use; 5. That in consideration thereto it is mandatory that the lot be occupied; 6. That up to the present since the inception of the HOA, I have not occupied the lot; 7. That the HOA already gave consideration for me to occupy the lot and ultimatum already handed out; 8. That it is mandatory that the lot be occupied to achieve the purpose of the Agreement and the spirit of the Community Mortgage Program (CMP) of the Social Housing Finance Corporation (SHFC); 9. That in consideration of the above premises and of my commitment to participate and cooperate with the HOA being a member-beneficiary thereof, I hereby undertake to occuply the lot assigned to me on _____________ without need of further and demand and grace period; 10. That failure on my part to do so will subject me to disciplinary measures as will be imposed by the said HOA in accordance with its By-Laws and other issuances and other existing laws, including but not limited to substitution and forfeiture of the lot assigned to me; 11. That I agree to the aforementioned disciplinary measures and all its ramifications and hold myself liable for perjury, falsehood, misrepresentation or omission, if I shall violate this Undertaking; and 12. That I attest to the truthfulness of this undertaking and I submit to the legal and administrative consequences thereof if ever the statements above are wanting in truth and in substance.
