Upon recommendation of the University of the Pacific, Arthur A. Dugoni School of Dentistrys
Faculty Appointment, Promotion, and Tenure Committee I am pleased to inform you of your
appointment as Adjunct Instructor in the Department of Periodontics effective February 14, 2017
scheduled to continue to August 31, 2017. Along with President Pamela A. Eibeck and Provost
Maria G. Pallavicini, I welcome you to the University of the Pacific.

Your agreement to accept this appointment is a commitment to serve the distinctive mission of the
University of the Pacific, in particular the Arthur A. Dugoni School of Dentistry, which values its
humanism, innovation, leadership, reflection, stewardship, collaboration and philanthropy. This
letter incorporates the policies and procedures applicable to your category of faculty appointment
as now contained in the Faculty Handbook. As the University may revise the handbook from time
to time, please refer to the University Faculty Handbook on the University website at

Your appointment is that of an unpaid volunteer as defined by law, without expectation of any
compensation or benefits, or other consideration for volunteer work. While you are a volunteer
and not an employee, you are not covered by workers compensation insurance; however you have
agreed to conform to University policies and procedures, particularly those related to security,
safety, professional behavior and prohibition of sexual and other unlawful harassment.

The terms of this offer are as follows:

1. You have volunteered to provide the following level of voluntary service: 1 day per week, 14
weeks per year.

2. This appointment becomes effective on February 14, 2017. This appointment continues until
the above scheduled end date and thereafter except in the event of failure to maintain active
license, special permit and other terms for compliance with the California Dental Practice Act,
for misconduct, or for unsatisfactory performance, it may be terminated sooner by the Dean.

3. Your service under this appointment does not count as time towards tenure eligibility at the
University of the Pacific.

4. Adjunct faculty members are not eligible for enrollment in the any fringe benefit programs
offered by the University.
5. The University requires all faculty to submit to a background check. This offer of voluntary
service is conditional on satisfactory completion of a background check. Please contact the
Human Resources Office at (415) 929-6454 if you have any questions regarding this process.

6. Please return the enclosed forms so that we have accurate address and emergency contact
information on file.

Volunteer faculty members make a significant contribution to our teaching program. On behalf of
the entire faculty, staff and students, I want to thank you for making your talents, time and abilities
available to us. I know that your contributions to our students will be of significance in the further
development of our educational programs.
