Personal training is one on one training where a Certified Personal Trainer devotes 100% of
their attention towards educating, motivating, and supervising your individually tailored
workout. A personal trainer will also help you to maintain the discipline and motivation
necessary to carry you through your workout on a consistent basis.
Individual exercisers find personal training valuable for many different reasons. In most cases,
you will retain a Personal Trainer to ensure proper training techniques in an effort to reduce the
risk of personal injury and maximize the benefits of your exercises.
Each session lasts one hour and can be scheduled according to each individuals goals or needs.
Meet with our Nutritionist to focus on your needs to create a diet and a weight loss plan to help
you reach your goals.
Lose weight and keep it off! You are instructed on how to reduce your caloric intake and how to
maintain healthier eating habits for long-term effects.
The nutritionist will meet with you frequently throughout the program to monitor your progress,
offer encouragement, and recommend additional dietary changes to help you achieve your
weight goals.
A health risk assessment will tell you what your present level of fitness is, help you to set goals
and have the ideal program designed to help you achieve these goals.
You will meet with a mature, experienced Fitness Appraiser who will explore your health and
fitness experience and assess your current fitness level to determine the following;
Flexibility, Cardio-Respiratory Fitness, Strength, Blood Pressure and Resting Heart Rate,
Percentage of Body Fat, Girth (body) measurements, Height and Weight.
The information will be processed and you will be given a full report of your results. Your
appraiser will help you set goals and put together a personalized program to help you achieve
your goals and improve your overall fitness level. If you suffer from a particular injury or
recovering from surgery, the appraiser will focus on your special training and rehabilitation
needs.
The Health Risk Assessment is a great way to get started on the right track. You will learn how
to make the best use of your time at the gym, getting better value from your membership in a
safe and competent manner.

You will first go through a physical assessment. You will then receive an initial one on one
training course on Why, What, and How weight gain happens and how you will use this
knowledge to lose weight.
Each week you will meet with a personal trainer for a 60-min one-on-one training session. Your
trainer will also motivate you to participate in existing club activities, stick with the weight loss
program and complete your exercises.
You will also meet with a nutritionist each week to ensure you are instructed on how to reduce
your caloric intake and will meet with you once per week throughout the program to review
your progress.
If you follow our program: you will lose weight, improve your health, and receive weight loss
and exercise coaching to continue through the rest of your life knowing what you need to do to
maintain a healthy weight.
This Program is especially right for you if you are joining Fitness Plus with weight loss in mind
and/or have previously tried other methods that either didnt work or only worked for a short

12 Month Memberships can be placed on HOLD for a $35 fee and may remain on hold for a maximum of
one year.
Payments for 12 Month Memberships paid monthly will continue to completion even if membership is
placed on HOLD.
The fee for monthly payment plan is based on the payment of a two (2) month installment and ten (10) post-
dated payments.
Perpetual Memberships come with a price guarantee and can be cancelled at any time with a 30-day
signed notice.
Pre-authorized payment memberships will be automatically renewed following the 12 month period.
Once renewed, the membership becomes perpetual unless instructed otherwise by the member.
Multiple Visit Passes expire one year from the date of purchase.
Personal Training, Physiotherapy, Massage Therapy or any other Personal Services Sessions must be used
within one year of purchase.
Appointment cancellations must be made 24 hours in advance or the appointment will be forfeited.

Member acknowledges that surveillance cameras are in use for the protection of the Facility, its
equipment, and its members. Member hereby consents to being photographed and/or recorded for such
purposes and acknowledges and agrees that such recordings may be used by Fitness Plus to ensure
compliance with this Agreement and may be shared by Fitness Plus with legal authorities in the event
of any crime taking place in or around the Facility.
Member also acknowledges that Fitness Plus operates under rules and regulations established for the
safety and protection of its members, and agrees to be bound by such rules and regulations, as well by
the rules and regulations subsequently approved and posted or otherwise published by Fitness Plus.
Fitness Plus reserves the right at any time to change its hours of operation, and to change the cost of,
and, modify or eliminate any program, equipment or class offered at the Facility. In addition,
regulations and policies are subject to change from time to time, without prior notice, in the sole
discretion of Fitness Plus. Member agrees to accept such reasonable change(s) as a condition of
membership.
In the event of membership price increases, Members will be notified and will have 30 days to respond.
No response during this time will be assumed as consent and agreement to proceed with the payments
at the increased price. Failure to abide by the rules and regulations may result in Members expulsion
from the Facility and termination of this Agreement .
KEY FOBS:
Key fobs are mandatory and assigned to members for their use only. Members are not to give their key
fob to anyone and are not to use their key fob to allow anyone else access to the Facility. Members are
reminded that there is no child care provided and as such, should make alternative arrangements.
Fitness Plus is a fitness facility; children, friends, and other family are prohibited from waiting for the
duration of Member's workout. When Member scans key fob, they should be aware of their
surroundings, especially during unstaffed hours, to prevent anyone else entering the Facility. The cost
is $50 and it is 100% refundable; return the key fob at the end of your membership and get your $50
back (does not apply to Bronze After Hours membership). Your key fob will automatically be
deactivated at the end of your membership.
If you do not need assistance and/or accidentally activate the device, just inform the Lifeline personnel
that it was an accident and you do not require an ambulance. Lifeline necklace are to remain in the
Facility and returned to the designated safety station after use. Misuse of Lifeline necklaces,
specifically resulting in an unnecessary ambulance dispatch, is subject to a $600 fine.
PROHIBITED ACTIVITIES:
Alcohol, drugs (including steroids), and smoking are prohibited within the Facility. Member agrees not
to use the Facility or engage in any activity at Fitness Plus while under the influence of drugs, alcohol,
or medication that may impair Member's ability to operate the equipment. No weapons of any kind are
allowed. No photography, videotaping, filming or audio recording is permitted within the Facility
without the express written consent of Fitness Plus' management. Fitness Plus reserves the right, in its
sole discretion, to limit the consumption of food or beverages (except water), or to the use of outside
equipment within the Facility. Gambling or gaming is prohibited within the Facility or on the premises.
