REPORTING DAMAGE &
HANDOVER NOTES

On a routine inspection of a marine department owned vessel, a large crack in the


vessels hull was found on the port quarter. The damage affected the watertight
integrity of the boat. The damage sustained had occurred about one month
previously but had not been reported.
The contractor crew was involved in the investigation but it was found that this
particular damage was incurred before they joined the vessel. The previous
Captain should have reported the incident immediately. If the incident had
been reported on time, costs could have been avoided and the overall safety of
the vessel would have been maintained. In failing to perform his required duty, the
previous Captain has been removed from the approved list of Captains and is no
longer suitable for any employment with Saudi Aramco.
The Incident Investigation Team found that the following procedures were not
followed:
MIM 1187.001 Collision and Damage by or to Marine Equipment or Craft.
MIM1185.002 section 1.16 on Hand Over Notes - The Master of each
vessel must ensure that clear and precise Handover Notes are written by
the departing Master to hand to the relieving Master. Further that the
departing Chief Engineer prepare Handover Notes to be handed over to the
relieving Chief Engineer. A record of all Handover Notes is to be kept in an
appropriate file by the Master of each vessel. A copy of the handover notes
must be sent to the operating superintendent.
Timely Vessel Inspections and Reporting Procedures in accordance with
Operating Standards as stipulated in Section 6.0 of the Marine Department
Loss Prevention Program.
