RE: Permission to Conduct an Interview

Dear Sir/Madam:

We are writing to request permission to conduct an interview at your center. We are

currently enrolled in Mechanical Engineering at Cebu Institute of Technology - University
and we are in the process of writing our thesis. The study is entitled Supportive Transport
for Elderly and Paralytic Persons (STEPP). We humbly request that the you will allow us
to interview you constituents so we can gather pertinent data about our study.

If approval is granted, we will have to conduct the interview personally. The interview
process should take no longer than ten minutes. The interview results will be pooled for
the thesis project and individual results of this study will remain absolutely confidential
and anonymous. Should this study be published, only pooled results will be documented.
No costs will be incurred by either your center or the individual participants.

Your approval to conduct this interview will be greatly appreciated.
