You are being given the opportunity to evaluate the mistakes you made on a test. Rework the
problems that you missed to improve the grade that you earned for that test. You must bring this form
completed to retake the test along with a sheet proving you got 50% or higher on all assignments
leading up to this test (ex. Youre trying to retake unit 2 test, all unit 2 assignments must have at least
5/10 points to retake), you can print this off of skyward.

The following must be done to be eligible for the re-test.

1. Rework all incorrect problems. You may use your book and class notes, friends, internet, etc.
2. Write the specific reason you missed the problem. (mis-read directions, used the wrong
formula, mis-multiplied left off units or wrong units, etc. You cannot say IDK or didnt
understand. What didnt you understand/know?) MUST BE SPECIFIC
3. Rework the problem showing all steps.
4. Write the new answer clearly in the answer column.
5. Additional corrections may be done on notebook paper using the same format shown below
or you may print additional copies.
6. Have your Parent Sign the front of this sheet.
7. Attach your TEST RETAKE FORM work to your ORIGINIAL TEST and the GRADE REPORT showing
50% or higher on all assignments for that unit.
8. After I have gone over the corrections with you, THEN you can retake the test.
