Definition of Anemia
B. Etiology of Anemia
Anemia is caused by a variety of illnesses, but all the damage would significantly
reduce the amount of oxygen available to the network. According to Brunner and Suddart
(2001), some of the general causes of anemia include:

d. Other factors include blood loss, nutritional deficiencies, hereditary factors, chronic
disease and iron deficiency.

D. Pathophysiology
Incidence of anemia reflects marrow failure or loss of red blood cells to excess or
both. marrow failure may occur due to nutritional deficiencies, toxic exposure, tumor
invasion or mostly due to unknown causes. red blood cells can be lost through bleeding or
hemplisis (destruction), this can be due to a defect of red blood cells that do not comply
with the resistance of red blood cells which causes destruction of red blood cells.
Red blood cell lysis (dissolution) occurs mainly in phagocytic cells or in the
reticuloendothelial system, mainly in the liver and spleen. Byproducts of this process is
bilirubin that would enter the bloodstream. every increase in red blood cell destruction
(hemolysis) immediately reflected by increased plasma bilirubin (normal concentrations 1
mg / dl, levels above 1.5 mg / dl lead to jaundice in the sclera).
When experiencing the destruction of red blood cells in circulation, (in hemolytic
disorders) it will appear in the plasma hemoglobin (hemoglobinemia). If the plasma
concentration exceeds capacity of plasma haptoglobin (protein binding to free hemoglobin)
to tie it all, hemoglobin diffuses in the kidney glomerulus and into urine (hemoglobinuria).

A. Classification of Anemia
Classification based on physiological approach:
1. Anemia hipoproliferatif, namely deficiency anemia the number of red blood cells
caused by a defect in the production of red blood cells, include:
a. Aplastic Anemia

The cause is the reduced survival of red blood cells and deficiency eritopoitin

Asupan besi tidak adekuat, kebutuhan meningkat selama hamil, menstruasi
gangguan eritropoesis
Gejala-gejalanya:

2. Anemia hemolitika, yaitu anemia defisiensi jumlah sel darah merah disebabkan oleh
destruksi sel darah merah:

e. Hemoglobin menurun
f. Rasio hemoglobin porfirin eritrosit ---- lebih dari 2,8 ug/g adalah diagnostic untuk
defisiensi besi
g. Mean cospuscle volume ( MCV) dan mean cospuscle hemoglobin concentration ( MCHC )
---- menurun menyebabkan anemia hipokrom mikrositik atau sel-sel darah merah yang
kecil-kecil dan pucat.
h. Selama pengobatan jumlah retikulosit ---- meningkat dalam 3 sampai 5 hari sesuadh
dimulainya terapi besi mengindikasikan respons terapeutik yang positif.
C. Penatalaksanaan Anemia
1. Anemia aplastik:
o Pada paien dialisis harus ditangani dengan pemberian besi dan asam folat
Dengan pemberian makanan yang adekuat. Pada defisiensi besi diberikan sulfas
ferosus 3 x 10 mg/hari. Transfusi darah diberikan bila kadar Hb kurang dari 5 gr %.
Pada defisiensi asam folat diberikan asam folat 3 x 5 mg/hari.
5. Anemia megaloblastik
o Defisiensi vitamin B12 ditangani dengan pemberian vitamin B12, bila difisiensi
disebabkan oleh defekabsorbsi atau tidak tersedianya faktor intrinsik dapat
diberikan vitamin B12 dengan injeksi IM.
o Untuk mencegah kekambuhan anemia terapi vitamin B12 harus diteruskan selama
hidup pasien yang menderita anemia pernisiosa atau malabsorbsi yang tidak dapat
dikoreksi.
o Anemia defisiensi asam folat penanganannya dengan diet dan penambahan asam
folat 1 mg/hari, secara IM pada pasien dengan gangguan absorbsi.
Dengan memberikan transfusi darah dan plasma. Dalam keadaan darurat diberikan
cairan intravena dengan cairan infus apa saja yang tersedia.
