Athletic Shirt Order Form

Cross country is a no-cut sport. This means we get upwards of eighty kids coming out to run. We
have a very tight budget that we operate on, but would like to continue to make this a sport that
is open and available to all grades and all runners.

With that in mind, we are offering a very cool Titan parent spirit shirt for $15.00. Show your
support for your Titan athlete by ordering one of these and wearing them to the next football
game, cross country race, or volleyball game. It is NOT sport specific, and perfect for cheering
on your Goddard athlete.

Please return this form, along with your check (made out to Goddard Middle School) or cash to
the front office or Mrs. Fry in Room 108 by Thursday, September 28. All proceeds beyond the
cost of the shirt will go to help the cross country team pay for assistant coaches as well as a
timing clock for races.

Shirts will be delivered approximately two weeks after the date above to your students sixth
period class.
