2012 YOUTH ADOLESCENT QUESTIONNAIRE (YAQ) ANNOUNCEMENT

NEW UPDATED QUESTIONNAIRE

YAQ. PDF. The new questionnaire is based on our latest 2011 GROWING UP TODAY
STUDY QUESTIONNAIRE and has been updated to better reflect the current market
place. We hope you will find the changes satisfactory.

The new FFQS are designed to replace the 1995 YAQ. The old questionnaire will be
available for past users of the questionnaire ONLY who will be completing their study
within the next 5 years. All new Questionnaire users and studies must use the 2012 YAQ.
We are in the process of developing a new scanning and analysis program. We will not be
able to provide analysis of New 2012 YAQs until this work is complete. We will try to
expedite this process and diminish any inconvenience to users.

In order for you to be aware of the differences between the old and the new FFQS we
have posted a file to the website:

Billing for ordering YAQ FFQS has been updated with new YAQ option.

Please note, we will continue to process the old YAQ ffqs for approximately 5 years. So if
you have old YAQS ready to process, please submit. Ultimately, we will stop maintaining
the analysis program for the old YAQ and only maintain the 2012 YAQ program.
