This root cause analysis tkely tobe a group acti where the root cause methodology is applied and the root causes taking account ofthe measurement evidence aeady available and aso the stay. The outcome expected from this stage is an agreement on key “foot causes" from allteam members, Once ‘completed this team then need to writeup ths stage oftheir work demonstrating that it has been a systematic approach which followed logical fom the measurement stage, “The nest stage requires the team to develop very specic actions for indidual inthe ‘company to best adcress the specic rot causes already agreed in the analysis. These ‘should sso be agreed by al team members and the actions should be “robust” i.e. the {eam should be able to defend these asthe most appropriate specific actons needed row. Ths stage then needs tobe written up, ‘The tean then need to evaluate the impact of ther improvement proposals (f Implemented) & compare this wth their inital goal. It should be made clear tothe reader (the MO! the impact of the specific proposals being made. Note: The MD is not interested ina largo number of proposals. He needs to have al the main proposals (ypcally 5-8 max) to deal with the major underying root causes. Finally, the team need to ensure thee reports professionally organised and they need to plan the optimum side sequence and prepare the sides necessary fora professional presentation. A professional presentation should bring clarity tothe listener and show logical follow-up through of ideas. the Report On A: “There ae four wocks availabe to complet the assignment.
