Person Centered Plan

Introduction:

Maggie King is a 10-year-old Caucasian female. She is currently in fifth grade

at Western Elementary School. Maggie has been receiving services to help assist her in

school since 3rd grade. She has an IEP and the classification listed is learning disability.

She is in an inclusive classroom setting and receives a variety of services, both push in

and pull out. She receives AIS reading services, resource room for extra support in all

subjects, and testing modifications. Specifically, the student struggles with reading

comprehension and spelling. She also receives weekly counseling sessions to help her

cope with her anxiety. In these sessions she learns strategies to self-monitor, stay

organized and allow her to express her emotions in a trusted environment.

Maggie is an only child and lives with her mom and dad. They live in a rural

neighborhood. She is also very close with her grandparents and cousins. She values

family and enjoys spending time with her loved ones. Maggie also loves to share with

her peers, stories about her fun adventures with her family.

In her free time Maggie enjoys outdoor activities such as kayaking and skiing.

She participates in the school ski club on the weekends. She also takes interest in

nature, specifically gardening. She loves reading, especially books about animals and

nature. She is a member of the local girl scout troop. Maggie has a close knit group of

friends consisting of girls from school as well as her outside activities. Maggie is a kind

and compassionate young girl. While she is shy, she does enjoy being in the company

of others and always wants to ensure everyone around her is safe, happy and healthy.

As Maggie plans to prepare for Middle School, she has decided that she would

like some support from her teachers, counselor, family and friends to help her with this

big transition. As she begins to think about this big change, she finds herself becoming

anxious and feels uneasy about what is to come. Maggies desire to please everyone

and be perfect, has created additional worries in her young life. She fears that she will

not be successful, make friends, be able to achieve good grades and keep her parents

happy.

In order to help Maggie, we have decided to create an action plan that will help

guide her and provide a tool to self-monitor when she finds herself feeling anxious or

unprepared. The plan we chose to enact will outline Maggies life. We will focus on her

strengths, gifts and talents, allowing her to see all that she is capable of and what

makes her so special. We will discuss her story, and all of those people available to her

that want to help her and be there to support her. Together we have outlined her goals

and dreams for the near future, to help all of us gain a better understanding of what she

hopes to accomplish. We have also discussed her worries and fears, to ensure that we

understand those things that make her upset and uneasy. Lastly, we have outlined a

plan of action. This plan will focus on how Maggie can help herself, but also, how all of

those in the meeting will help her and guide her.

Provide a reflection of the meeting in which you discuss your personal reaction to

the process itself along with any discoveries about the individual. Describe the process

Overall the meeting was very successful and everyone contributed some great

insight and support for Maggie. Since meeting, Maggie has expressed that she is happy

we held the meeting and feels better prepared for what is to come. During the meeting

Maggie really opened up about her struggles and fears lately. She was very honest and

seemed comfortable sharing her personal life with those in the meeting, it was a very

honest and trusting environment.

The process was very organized and successful. We were able to have all of the

people in Maggies closest circles attend. These individuals brought a lot of great

information and support. Each other these individuals now has a developed and

outlined role of how they will help us execute Maggies plan within the coming months.

We were able to look at where we still need to pull in additional resources and schedule

another time to meet and reassess the progress and success of her plan. Maggie was

able to have 100% input on the choices that were made and the developed plan. She

seemed satisfied with the outcome as did the members in the meeting.
During the meeting we really discovered Maggies sentiments towards this big

transition in her life. Some of us were shocked at how much she feared this exciting

change and we now have a better understanding of how to address it and where we

need to move forward from here. This was a great opportunity to get everyone together

and talk about things that are going really well and things that we can continue to work

on together to help Maggie feel confident, independent and prepared for lifes changes!
