my/our ownership and understand that all associated and medical activities and

fees regarding ________________________________ are now our/my responsibility

beginning this day ____________________________, I understand that is it my responsibility

to provide a safe and nurturing environment for _____________________________, free from

neglect an abuse. Initial __________


I, ___________________________, also understand that it can take weeks or even months for
a pet to become comfortable to adjust to their new home environment. Though if
there are concerns, I/we swear to contact Jenni Riha at the Foster Kitten Project
PRIOR to ever submitting to a shelter!

I, _____________________________, also understand that by signing this agreement I agree
to have this kitten in which I am adopting spayed or neutered and up to date on all
vaccines and general healthcare throughout their lifetime. By not agreeing to these
terms will void this adoption agreement. I also understand that providing proof of
these procedures may be requested at anytime by the Foster Kitten Project and I
agree to provide copies of those documents if requested.

Jenni Riha and The Foster Kitten Project is no longer the foster caregiver of

___________________________, and all associated social and medical activities and fees

pertaining to them beginning this day ______________________________ are relinquished.
