AFFIDAVIT

I, ___________________, of legal age, married, Filipino, and a

resident of _______________, after being duly sworn to in accordance
with law, depose and say that:

1. On ________, Cheryl Loberiano leased a unit in my apartment

with a monthly rental of ______;

2. Before her occupancy, she intimated to me and asked for my

permission that a certain Raymond Gidal, whom she introduced
as her boyfriend, be allowed to stay in her apartment unit;

3. Based on what Ive observed during the first couple of months

of their occupancy, I considered them as live-in partners due
to the fact that Gidal used to stay in her apartment unit for two
or three weeks in a month;

4. Sometime in ____, Gidal and Loberiano started quarrelling

wherein the former started abusing, verbally and physically,
the latter;

5. After committing violence upon the person of Cheryl, Gidal

used to walk away and leave their unit;

6. After few days from leaving their apartment unit, Gidal will
return and beg for forgiveness for what hed done;

7. I witnessed and can attest to the sufferings that Cheryl went

through in the hands of Raymond.

8. On ______, our lease contract ended and Cheryl vacated her

apartment unit;

9. I am executing this affidavit to prove the truthfulness and

veracity of the foregoing statement.
