Subject: Charge-Sheet

1) You are employed in the Company as ____________________.

2) You are remaining absent unauthorizedly without permission and/or

intimation from/to your superiors w.e.f. 28.01.2017 till date.

3) Your above-mentioned act of remaining absent without permission

and/or intimation/ authorization has caused and is causing tremendous
inconvenience, hardship and prejudice to the Company, in addition to
affecting the working of the Company.

4) You were therefore addressed a Charge-Sheet dated __________ calling

upon you to show-cause in writing within a period of 3 days from the
date of issue of the said Show Cause Notice as to why action as
prescribed in law should not be taken against you in respect of your
unauthorized absence and your unauthorized collection of the said
amount of Rs. 11,00,000.00.

5) You are therefore hereby being Charge-Sheeted for the above acts, which,
if proved, would amount to the following Misconducts under the Model
Standing Orders framed under the Industrial Employment (Standing
Orders) Act, 1946:

24 (a) willful insubordination or disobedience of any lawful or reasonable

order of a superior;

24 (d) theft, fraud or dishonesty in connection with the employers

business;
 24 (f) habitual absence without leave; absence without leave for more

than ten consecutive days;

24 (l) commission of any act subversive of discipline or good behavior;

A Domestic Enquiry in the matter of the above charges leveled against you will

be conducted against you and the first sitting will be held on ______________ at

__________ p.m. in the Office of the Company situated at

___________________________.

You will be permitted to file documents and examine your witnesses in your

defense if you so desire. You will be permitted to be represented in the Enquiry

by any of your co-workmen if you so desire. You will also be given an

opportunity to cross-examine the witnesses examined on behalf of the

management.

Please take note that if you do not attend the Enquiry on the date, time and

place mentioned above, the Management will press before the Enquiry Officer

to proceed with the enquiry ex-parte without any further intimation to you.
