NOTICE TO SHOW CAUSE

We regret to inform you that the Company has received allegations of misconduct on your part
and have commenced an investigation into the same. Whilst we conduct our own internal
investigations, we would like you to explain to the Management in writing your explanation on
the allegations stated from (1) to (2) below.

It is alleged that you in breach of your express and/or implied terms and conditions of service,
including the Company’s Human Resources Manual, the Department’s Standard Operating
Procedures and the Notice from the Department Head dated ….date….., have committed the
following acts of misconduct, the particulars/details of which are as enumerated below :-

By so doing, you had:

(a) acted in a negligent manner; and/or

(b) abused your position of trust and authority in the Company; and/or

By so doing, you had:
(a) acted in a negligent manner; and/or

(b) acted in a manner incompatible with your position of trust and authority in the Company;

thereby committing a serious act of misconduct.

The allegations of misconduct leveled against you as stated above, if found to be true,
constitute serious misconduct on your part and accordingly you will be liable to severe
disciplinary action, including the punishment of dismissal from service. Therefore, it is
imperative that you provide your written explanation to the above allegations of misconduct
levelled against you, and show cause as to why disciplinary action should not be taken against
you in respect of the said allegations of misconduct.

Your written explanation to the above stated allegation should be received by the undersigned
by 5.00 p.m. on [please state date within 3 working days of date of letter], failing which the
Company will assume that you have no explanation to offer and will proceed to act accordingly,
without further notice to you.

In view of the serious nature of the allegations of misconduct made against you and to facilitate
the Company’s investigations into the said allegations of misconduct and any other matters that
may be connected with your conduct as an employee of the Company, you are hereby
suspended from service on full pay with immediate effect, pending the receipt of your reply
and/or the completion of the Company’s investigations and/or the outcome of any inquiry which
the Company may wish to conduct against you.

During the period of your suspension, you are strictly not authorized to enter the premises of the
Company, without the express written approval of the undersigned. You are further strictly not
authorized to conduct any business on behalf of the Company, and/or contact any employees of
the Company and/or customers, vendors, suppliers and potential employees, and/or business
associates of the Company, without the express written approval of the undersigned. Should
you have any queries or require clarification regarding this strict prohibition, please do not
hesitate to contact the undersigned.

During normal working hours you are required to remain contactable to enable the Company to
contact you in the event your assistance in the investigations is required. In this regard, you are
required to furnish details of your contact number(s) and your latest address for ease of
communication.

Kindly acknowledge receipt of this letter by attaching your signature on the duplicate and
returning the same for the Company’s records.
