The Board of Elections in the City of New York has reason to believe that you are no longer
qualified to be a registered voter in the City of New York. The Board of Elections believes that
it may be required to cancel your registration.
If you do not appear or answer by mail within 14 days after the mailing of this notice, your
registration will be cancelled. All written responses must be made in English.
However, after the cancellation of your registration, you may register pursuant to the provisions
of Title IV, Section 5-400 through 5-404 of the State of New York Election Law. For
information on this procedure, including days and hours of local registration and the location of
local registration places, the deadline for personal registration by mail, call 866-VOTE-NYC or
visit our Web site, wwrv.vote.nvc.ny.us.
