Youth Commitment Form

The Senior High Youth of Third Church will be traveling to and participating
in Montreat Youth Conference week 3, July 6 - 14, 2018. This trip is open to
all current 9th-12th grade youth. The cost of this trip is approximately $500
per person. A non-refundable $250 deposit is due February 1st. The
remaining $250 is due April 1st.

By signing this form, Parents and Youth are committing to participating in

the trip and in taking part in fundraising efforts.
