LETTER OF CONSENT

TO WHOM IT MAY CONCERN:

_____________________________(Student), DO HEREBY GIVE OUR CONSENT for him/her to attend
the gift giving as part of the thanks giving celebration of our section on December 17, 2019, a day after
Chrismas party. Inherent risks are associated with any activity and by granting permission for my
son/daughter to participate, I acknowledge that such risks exist. However, I believe that the essense of
this celebration is sharing love and helping people and this opportunity outweighs these risks and I
hereby grant permission for my son/daughter to participate.
