AFFIDAVIT OF DESISTANCE

I________________, of legal age, Filipino, single and with office address at

___________________________________________________after having been duly
sworn to in accordance with law, hereby depose and say:

1. I am one of the Legal Officers of____________, a domestic corporation duly

organized and created under and by virtue of the existing laws of the
Republic of the Philippines with principal place of business at
Unit____________________________, Philippines;

2. I am duly authorized to represent and sign in behalf of ___and to institute

civil and/or criminal complaint against any person/borrower in pursuant
to Board Resolution No. 2017-0009 approved on July 6, 2017. Machine
copy of which is hereto attached and marked as Annex A and made an
integral part hereof;

3. That Criminal Case number ___________entitled People of the Philippines

versus _____________for Violation of Batas Pambansa Bilang 22 is pending
for trial and resolution before this Municipal Trial Court in Cities
(Cebu City, Philippines.

4. That accused has already paid in full of the amount of the check subject
in the above-mentioned case.

5. That in view thereof, private complainant thru the undersigned authorized

representative would like to inform this Honorable Court that complainant
is no longer interested in the prosecution of the above-mentioned case and
respectfully move for the dismissal of the same against the accused.
