Abstract
Monoclonal gammopathy of renal significance (MGRS) is a recent and novel entity in monoclo-nal gammopathy of
undetermined significance (MGUS). Crystalglobulinemia syndrome (CS), which commonly contributes to multiple
mye-loma (MM)-related kidney diseases, can occur in MGUS. Therefore, it is important for physicians to be aware of
MGRS due to CS. Case presentation An 86-year-old woman presented with fever, paresis and dysarthria. Although a
vascular disorder was suspected in the central nervous system (CNS), mag-netic resonance imaging of the head was
normal. Her leukocyte count was 12.9109/l, hemoglobin concentration 11.4 g/dl and platelet count 210.0 109/l. Blood
chemistry was as follows: C-reactive protein 18.2mg/dl, blood urea nitrogen 53.3mg/dl, creatinine 2.4mg/dl and
