PRIZE ACCEPTANCE AGREEMENT

This Prize Acceptance Agreement must be fully completed, signed, and returned within three (3) days of
receipt, or the prize may be forfeited and awarded to an alternate winner. This Prize Acceptance
Agreement is considered received on the date it is emailed to you.

I am submitting this Prize Acceptance Agreement to confirm the terms and conditions applicable to my receipt of the prize
in the Lucky6ix Sweepstakes sponsored by the National Lottery and major email providers, in which I have been selected
as one (1) of the six (6) winners.

1. Prize. I understand that the prize amount won is £875,320.00 (Eight Hundred and Seventy-Five Thousand,
Three Hundred and Twenty Pounds). The right to receive the prize is not transferable. My prize will not be paid
or given to anyone acting on my behalf.

2. Age and Eligibility. I am eighteen (18) years of age or older. I am not an employee, officer or director of Lucky6ix
or any of its advertising agencies or other entity involved in the development, production, administration,
sponsorship or fulfillment of the sweepstakes, or an immediate family member of any such person.

3. Costs and Expenses. I understand that if I am unable to personally claim the prize Over-The-Counter at the
designated Claim Centre, Lucky6ix will not be responsible for any delivery costs which I may incur in connection
with my Prize. I understand that I am solely responsible for any and all federal, state and local taxes, fees and
assessments of any governmental authority (including, without limitation, income taxes) on the prize that I am
awarded and for obtaining my own tax or legal advisor in connection with such obligation.

4. Charity. I understand that 40% of the prize I am awarded must be spent on a charity organization of my choice
and I can decide to start my own charity organization if I so wish. I agree to provide evidence of my expenses
one (1) year after I have received my prize. Acceptable proof of expenses are invoices, receipts, bank
statements, logs and any other financial documentation that evidences my expenses.

5. Claim Period. I understand that the prize must be claimed within fourteen (14) days of receiving this agreement
and instructions on how to claim my prize. If a cheque is issued to me, it is expected to be deposited into my
account within the claim period. If after the claim period my prize has not been claimed or the prize cheque has
not been received or deposited, the prize will be cancelled and forfeited.

6. Accuracy of Statements. I agree that if any statement made by me in this Prize Acceptance Agreement is false,
then, in addition to any other remedies that may be taken against me, I will forfeit my prize and if already
received by me, I will immediately return upon demand the prize that has been awarded to me.
