CLINICAL ORTHO/DIAGNOSIS 2/7/17

Overview:
» Ortho cannot be done with ankylosed teeth
» MDA is treated to the mandibular arch, not the maxillary
» MDA is only a 75% prediction – you can be wrong 25% of the time
o Most of the time it will work but not every time
» 2 ways to improve lower incisors
o you can go down – very hard
o you can go out  relative intrusion!
» If the molar relation is not class I (MB cusp does not fit into buccal groove) – measure the
difference and add together
o If you allow the molars to move forward, it takes up room

Diagnosis/treatment plan form:


» Physical growth percentiles – form in section F
o Men grow longer
o Girls & boys that mature early grow for a shorter amount of time and grow shorter
o In general, delayed growers will grow longer than the average
» Ortho dx form
o The number one cause for someone coming to the orthodontist is CROWDING
o Ortho deals with group function – because canines are not in place yet
o Facial angle – a hard tissue measurement, where is the mandible in relation to the face?
o POG is a secondary sex characteristics
o 4 specific ceph numbers we need to have down for the DIAGNOSIS
o Notes on classification of face and occlusion
▪ 1. Face
• soft tissue face (salmon form) – not used here
• during work up, youre using the ceph and using skeletal measurements
ptofile– this is the facial analysis to use here!
• Skeletal profile
o 85-90 non divergent; over 90 – overdivergent
o Convex (anything greater than 5), straight (+/- 0 - 5), or concave
(anything negative)
o On this child we have a mild, divergent…. Facial analysis
▪ 2. Occlusion – angles classification
▪ 3. Problem list – this list should be made right above this area where it asks “what
are the three most important factors in the dx”
• listed from most severe to least severe
▪ 4. State of dentition: primary, transitional, or permanent
o ALL radiographs are prescriptive – NOT ROUTINE
o We do a 1 week and 1 month check on all appliances
▪ Active appliances are every month
▪ Passive appliances are every 4 months
