A Novel Technology to Non-Invasively Measure

Continuous Cardiac Output from ECG and SpO₂
Estimated Continuous Cardiac Output (esCCO) is a new technology to determine
the cardiac output using Pulse Wave Transit Time (PWTT) which is obtained by
the pulse oximetry and ECG-signals from each cycle of the ECG and peripheral
pulse wave.
esCCOTM provides real-time, continuous and non-invasive cardiac output mea-

surement alongside the familiar vital sign parameter of ECG and SpO₂.
By only entering patient information such as age, gender, height and weight,
and an initial NIBP measurement, esCCOTM determines a reference value for ca-
libration and is ready for start the measurement.
Enhanced Hemodynamic Monitoring at No Extra Cost

Provided over the NIHON KOHDEN Patient Monitoring Series, esCCOTM repre-
sents a care enhancing and economic solution as no additional running costs

In 1984, NIHON KOHDEN implemented a comprehensive companywide quality system, IQC – Integrated Quality
Control. Under this system, quality begins at the earliest stage of every product’s life. Our IQC applies to the entire
manufacturing process, from the planning stage through product development, design, parts procurement,
production, inspection, storage, sales, distribution and after-sales support. IQC also involves nearly every employee.
Our rigid test procedures are structured to exceed the provisions of the GMP, Good Manufacturing Practice, codes
enforced by the US Food and Drug Administration. NIHON KOHDEN also has 130 in-house quality control inspection
staff to ensure that standards are implemented and maintained in every department of the company.

ECG Electrodes
NIHON KOHDEN VITRODE Disposable ECG Electrodes –
The result of over 30 years experience in disposable electrode manufacturing.
The ECG electrodes are the most important portions of the ECG/arrhythmia monitoring system. It is through these elec-
trodes that the electrical signals are conducted from the patient and into the monitoring system. If the electrodes are
left in place for an extended period of time (over 48 hours), or they are reused once they are removed, they do not ad-
here to the patient’s skin and they cause “noisy’’ ECG signals because of the poor conduction. In addition, the patient’s
movement causes the electrodes to lose conducting capabilities intermittently, and this leads to a “wandering’’ ECG
waveform and muscle artifact. The standard monitoring practice is to change these electrodes at least every 48 hours.

SpO₂ Measurement
A Pulse Oximeter is a non-invasive device used to percutaneously measure oxygen saturation
in the blood by use of a clip-on finger probe. Pulse oximeter works with the integration of a
probe placed onto patient’s fingertip or toe, obtains signals from the probe and the main
device calculates oxygen saturation in the body.
Oxygen saturation measured by pulse oximeter is SpO₂. “P” stands for “pulse”, and the mea-
surement principle is based on beat-by-beat changes in blood volume. In general, arterial
oxygen saturation measured by spectroscopic analysis is called “SaO₂”, and oxygen satura-
tion measured by pulse oximeter is called “SpO₂”.
The method for determination of arterial oxygen saturation, based on the photoelectric ple-
thysmography, was conceptualised and implemented in 1972 by the Japanese bio-engineer
Takuo Aoyagi (NIHON KOHDEN Corporation). This idea came about while using an ear den-
sitometer for recording dilution curves for dyes (pigments). The first commercial oximeter
“OLV-5100” was presented by NIHON KOHDEN back in 1975.
Dr. Aoyagi’s innovative technology, pulse oximetry, has rapidly spread worldwide and become essential in critical care. In
2002, Dr. Aoyagi was awarded the Medal of Honor in Japan, in recognition of his great contribution to clinical medicine.

NIHON KOHDEN reusable SpO2 finger-clips probes have paraellel mounted upper and lower sensor shells, thus, providing on the one hand, exact align-
ment of the optical sensor components vis-à-vis one another and on the other hand, markedly enhanced comfort during long-term attachment. In
addition, flexible lateral flaps shield the measuring signals from the external light, thus resulting to minimize the interference by additional artificial
light sources! Bright LEDs provide more accurate measurement even for thicker fingers or dark skin tones. The finger-clip reusable probe is the most
popular type of SpO2 probes. Instead of the typical hinge near the finger tip, P225F/P225H have a unique sure fitting and comfortable mechanism using
two sliding pieces for the front and back of the finger with two rubber flaps along both sides of the finger. When you press the button, the two halves
of the probe slide directly open, parallel to each other. No additional fixation tape is needed. For a stable and comfortable measurement P225F/P225H,
always keep the light emitters and detector aligned. NIHON KOHDEN provides unique reusable SpO2 probes.

Our special designed finger-tip probes, P310C and P311C, fit immediately on the finger and are perfect for long-term measurement. A less covered finger
area and cushioned materials lead to decrease pressure to the attachment site. The 3D molding attachment ensures a comfortable fit.
In combination with the soft and lightweight design the patient stress or discomfort is reduced and the probes can be remained up to 8 hours to one attach-

The multi-site probe P205A can be used for SpO2 measurement for children and adults as well as for prematures in combination with special attachment
tapes. For measuring SpO2 on children or adults with P205A, the ear clip P256 is required.
The ear clip is designed to hold the light emitters and detector of the probe P205A and be clipped onto the earlobe of the patient. P256 is intended for
use when SpO2 measurement on a finger or toe is difficult and the patient is at rest with little body movement. The P205A can be inserted in the special
designed slots on the ear clip without sticking out of the clip edge. A secure fit is provided by additional cable slots, silicone surfaces for the earlobe and
supplementary fixation of the cable with tape near the earlobe. The multi-site probe is intended for single-patient use and needs to be replaced after each
patient measurement. The ear clip can be reused. Clean it after every use with a soft cloth moistened with 70% ethanol solution. Dry the clip thoroughly
after cleaning.

Disposable SpO₂ Probes

NIHON KOHDEN disposable probes, from neonate to adult sizes, place special emphasis on the exact positioning of the LEDs and
detector. For this reason the two optical components are designed separately and can thus be brought into precise alignment with
each other by the user. All probes have breathable material and can be remained up to 8 hours on one attachment site. Paediatric
probes have a cute cartoon character printed on the tape to reassure children.
Self-adhesive, soft and thin tape provides a secure fit. The special design allows an easy application to the attachment site. To get
the most comfortable fit, please bend the probe before attaching, do not strongly pull the attachment tape when attaching and do
not push the tip of the finger with the attachment tape.

The probe P205A is specially designed for extremely sensitive skin. It uses soft and breathable sponge for the attachment tape. The attachment tape
and sensor parts are separate. You can select from two sizes of attachment tape: P260A or P260B. When using the probes without those tapes for
prematures, SpO2 measurement may be incorrect and the skin at the attachment site may be irritated.
The attachment tape consists of two parts: adhesive and non-adhesive part. To get best fit, place the tape at the required attachment site. Wrap
the sponge attachment tape (non-adhesive part) around the attachment site. Confirm that the light emitters and detector face to each other before
removing backing paper of the adhesive part. Then continue wrapping to fasten the probe. Replace the sponge attachment tapes every 24 hours.

Handling of the probe affects measurement accuracy and patient safety

For the proper placement of the probe on the finger, it is advisable that the light emitters and detector are placed just opposite to each
other. When using a clip-finger probe, it is necessary to ensure that the probe covers the finger properly, otherwise some portion of the
light never passes through the body tissues and may reach the light-receiving part directly, leading to incorrect measurement results.
The more distant the light-emitters and detector gets, the weaker the transmitted light intensity becomes. Additionally, a thinner body part
tends to have small amount of blood, wich makes it difficult to extract the arterial blood signal component. The most suitable thickness
of the probe site is about 10 mm.
Too loose probes may interfere with alignment of LEDs and detector, come off easily by body movement, cause larger motion artifacts or
be effected by venous pulse.
Too tight probes may cause venous pulsation, cause skin injuries such as burn and necrosis or unstable measurement because of a lower
pulsatile signal.

NIBP Measurement (Non-Invasive Blood Pressure)

Blood pressure (BP) is the pressure exerted by circulating blood against the artery walls. During each heartbeat, the blood
pressure varies between a maximum (systolic) and a minimum (diastolic) pressure. The variation indicates the condition
of the pumping function of the heart as an important vital sign.
A non-invasive indirect method to measure the blood pressure is the oscillometric blood pressure measurement. This me-
thod is using an inflated blood pressure cuff which is applied to the patient’s arm or thigh. The blood pressure is detected
by partial occlusion of the artery by the cuff. The so called Korotkoff sound is generated by the distension of the arterial
wall with each cardiac impulse. Systolic and diastolic blood pressures are determined by an algorithmic interpretation of
the device based on the mean arterial pressure and changing amplitude of the Korotkoff sounds.
For a proper non-invasive measurement it is important to use the correct cuff size due to the fact that a cuff too small will
lead to faulty high pressure, while a large cuft can lead to wrong low pressure readings.

Reusable YAWARA NIBP Cuffs

NIHON KOHDEN YAWARA-CUFF means soft and strong. The name YAWARA is based on the Japanese words YAWARAkai = Soft (in Japanese),
YAWARA Judo (in Japanese) and Judo means Strong.
The softness of the cuff is realized in the soft and smooth polyester with a buffer sheet over the skin contact area reducing the risk of wrinkling the
patient’s skin and the risk of internal bleeding. The buffer sheet also evenly distributes the air pressure to prevent too much pressure on one point.
The strongness is realized both in the nylon cable in the air tube preventing incidents caused by tube kinking and the durable bladder material
(polyurethane).
In addition to this the YAWARA-CUFF is characterized by easy handling. A wide opening lets you easily remove and insert the bladder for cleaning. The cloth is
water-resistant, so you can wipe it without any problem. The watertight connector cap prevents fluid from getting into the air tube while cleaning and is pro-
vided with each cuff. An outside printed cuff scale behind the arm helps you quickly check if the cuff has the right size for the patient. The upper arm should be
between the minimum and maximum scale.

The V910 airway adapter has an inner cylinder to reduce dead space. When this airway

inner cylinder. To eliminate this interference, use a V910 connector between the airway

BIS & TOF Measurement

The bispectral index (BIS) monitors the depth of anesthesia by processing raw EEG data, obtained through a sensor placed on
the patient’s forehead. A calculated number between 0 and 100 provides a direct measure of the patient’s level of consciousness
and response to sedation. Whereas a BIS value of 100 indicates the patient is fully awake, a value of 0 indicates the absence of
brain activity.
When admistering a neuromuscular blocking, TOF measurement should be used during all phases of general anesthesia from
induction to maintenance, and through to the end of a surgical procedure to monitor the degree of neuromuscular blockade. It
may help detect remnant blockades linked to airway problems, ventilator failure and further postoperative complications such
as severe hypoxemia or pneumonia.

TOF Application Help
Place the stimulation electrodes G210C over the Nerve Ulnaris on the volar side of the wrist. The distal electrode is positioned where the proximal
bending line crosses the radial side of the flexor carpi ulnaris muscle. Place the electrodes on each side of the expected position of the ulnar nerve
(2 to 3 cm distance). Softly press the electrodes to ensure good fitting.
Fix the transducer (is part of supply code NKD-79950213) to the thumb by putting the flat side to the thumb. Take care that no traction is applied
and that movement of the thumb is not obstructed. Small sideway movements of the thumb may be noticed during stimulation. If this is the case,
reposition the transducer in such a way that the movement is perpendicular. The more distal the transducer is placed on the thumb, the stronger the
acceleration signal is. This effect can be used to adjust the signal strength.
During the measurement, the arm should be immobile and fixed to the OR table or by the hand adapter NKD-79950209.

ECG Electrodes
NIHON KOHDEN VITRODE Disposable ECG Electrodes –
The result of over 30 years experience in disposable electrode manufacturing.
The ECG electrodes are the most important portions of the ECG/arrhythmia monitoring system. It is through these elec-
trodes that the electrical signals are conducted from the patient and into the monitoring system. If the electrodes are
left in place for an extended periode of time (over 48 hours), or they are reused once they are removed, they do not ad-
here to the patient’s skin and they causes “noisy’’ ECG signals because of the poor conduction. In addition, the patient’s
movement causes the electrodes to lose conducting capabilities intermittently, and this leads to a “wandering’’ ECG
waveform and muscle artifact. The standard monitoring practice is to change these electrodes at least every 48 hours.

SpO₂ Measurement
Pulse Oximeter is a non-invasive device used to percutaneously measure oxygen saturation
in the blood by use of a clip-on finger probe. Pulse oximeter works with the integration of
a probe placed onto patient’s fingertip or toe, obtains signals from the probe and the main
device calculates oxygen saturation in the body.
Oxygen saturation measured by pulse oximeter is SpO₂. “P” stands for “pulse”, and the mea-
surement principle is based on beat-by-beat changes in blood volume. In general, arterial
oxygen saturation measured by spectroscopic analysis is called “SaO₂”, and oxygen satura-
tion measured by pulse oximeter is called “SpO₂”.
The method for determination of arterial oxygen saturation, based on the photoelectric ple-
thysmography, was conceptualised and implemented in 1972 by the Japanese bio-engineer
Takuo Aoyagi (NIHON KOHDEN Corporation). This idea came about while using an ear den-
sitometer for recording dilution curves for dyes (pigments). The first commercial oximeter
“OLV-5100” was presented by NIHON KOHDEN back in 1975.
Dr. Aoyagi’s innovative technology, pulse oximetry, has rapidly spread worldwide and become essential in critical care. In
2002, Dr. Aoyagi was awarded the Medal of Honor in Japan, in recognition of his great contribution to clinical medicine.

NIHON KOHDEN reusable SpO2 finger-clips probes have paraellel mounted upper and lower sensor shells, thus, providing on the one hand,
exact alignment of the optical sensor components vis-à-vis one another and on the other hand, markedly enhanced comfort during long-term
attachment. In addition, flexible lateral flaps shield the measuring signals from the external light, thus resulting to minimize the interference
by additional artificial light sources! Bright LEDs provide more accurate measurement even for thicker fingers or dark skin tones. The finger-clip
reusable probe is the most popular type of SpO2 probes. Instead of the typical hinge near the finger tip, P225F/P225H have a unique sure fitting
and comfortable mechanism using two sliding pieces for the front and back of the finger with two rubber flaps along both sides of the finger.
When you press the button, the two halves of the probe slide directly open, parallel to each other. No additional fixation tape is needed. For a stable and
comfortable measurement P225F/P225H, always keep the light emitters and detector aligned. NIHON KOHDEN provides unique reusable SpO2 probes.

The multi-site probe P205A can be used for SpO2 measurement for children and adults as well as for prematures in combination with special attachment
tapes. For measuring SpO2 on children or adults with P205A, the ear clip P256 is required.
The ear clip is designed to hold the light emitters and detector of the probe P205A and be clipped onto the earlobe of the patient. P256 is intended for
use when SpO2 measurement on a finger or toe is difficult and the patient is at rest with little body movement. The P205A can be inserted in the special
designed slots on the ear clip without sticking out of the clip edge. A secure fit is provided by additional cable slots, silicone surfaces for the earlobe and
supplementary fixation of the cable with tape near the earlobe. The multi-site probe is intended for single-patient use and needs to be replaced after each
patient measurement. The ear clip can be reused. Clean it after every use with a soft cloth moistened with 70% ethanol solution. Dry the clip thoroughly
after cleaning.

NIHON KOHDEN disposable probes, from neonate to adult sizes, place special emphasis on the exact positioning of the LEDs and
detector. For this reason the two optical components are designed separately and can thus be brought into precise alignment with
each other by the user. All probes have breathable material and can be remained up to 8 hours on one attachment site. Paediatric
probes have a cute cartoon character printed on the tape to reassure children.
Self-adhesive, soft and thin tape provides a secure fit. The special design allows an easy application to the attachment site. To get
the most comfortable fit, please bend the probe before attaching, do not strongly pull the attachment tape when attaching and do
not push the tip of the finger with the attachment tape.

Facts
The probe P205A is specially designed for extremely sensitive skin. It uses soft and breathable sponge for the attachment tape. The attachment tape
and sensor parts are separate. You can select from two sizes of attachment tape: P260A or P260B. When using the probes without those tapes for
prematures, SpO2 measurement may be incorrect and the skin at the attachment site may be irritated.
The attachment tape consists of two parts: adhesive and non-adhesive part. To get best fit, place the tape at the required attachment site. Wrap
the sponge attachment tape (non-adhesive part) around the attachment site. Confirm that the light emitters and detector face to each other before
removing backing paper of the adhesive part. Then continue wrapping to fasten the probe. Replace the sponge attachment tapes every 24 hours.

Handling of the probe affects measurement accuracy and patient safety

For the proper placement of the probe on the finger, it is advisable that the light emitters and detector are placed just opposite to each
other. When using a clip-finger probe, it is necessary to ensure that the probe covers the finger properly, otherwise some portion of the
light never passes through the body tissues and may reach the light-receiving part directly, leading to incorrect measurement results.
The more distant the light-emitters and detector gets, the weaker the transmitted light intensity becomes. Additionally, a thinner body part
tends to have small amount of blood, wich makes it difficult to extract the arterial blood signal component. The most suitable thickness
of the probe site is about 10 mm.
Too loose probes may interfere with alignment of LEDs and detector, come off easily by body movement, cause larger motion artifacts or
be effected by venous pulse.
Too tight probes may cause venous pulsation, cause skin injuries such as burn and necrosis or unstable measurement because of a lower
pulsatile signal.
NIBP Measurement (Non-Invasvie Blood Pressure)

pressure varies between a maximum (systolic) and a minimum (diastolic) pressure. The variation indicates the condition
of the pumping function of the heart as an important vital sign.
A non-invasive indirect method to measure the blood pressure is the oscillometric blood pressure measurement. This me-
thod is using an inflated blood pressure cuff which is applied to the patient’s arm or thigh. The blood pressure is detected
by partial occlusion of the artery by the cuff. The so called Korotkoff sound is generated by the distension of the arterial
wall with each cardiac impulse. Systolic and diastolic blood pressures are determined by an algorithmic interpretation of
the device based on the mean arterial pressure and changing amplitude of the Korotkoff sounds.
For a proper non-invasive measurement it is important to use the correct cuff size due to the fact that a too small cuff will
lead to faulty high pressure, while a too large will lead to wrong low pressure readings.

Reusable YAWARA NIBP Cuffs
Facts
NIHON KOHDEN YAWARA-CUFF means soft and strong cuff. The name YAWARA is based on the Japanese words YAWARAkai = Soft (in Japanese),
YAWARA Judo (in Japanese) and Judo means Strong.
The softness of the cuff is realized in the soft and smooth polyester with a buffer sheet over the skin contact area reducing the risk of wrinkling the
patient’s skin and the risk of internal bleeding. The buffer sheet also evenly distributes the air pressure to prevent too much pressure on one point.
The strongness is realized both in the nylon cable in the air tube preventing incidents caused by tube kinking and the durable bladder material
(polyurethane).
In addition to this the YAWARA-CUFF is characterized by easy handling. A wide opening lets you easily remove and insert the bladder for cleaning. The cloth is
water-resistant, so you can wipe it without any problem. The watertight connector cap prevents fluid from getting into the air tube while cleaning and is pro-
vided with each cuff. An outside printed cuff scale behind the arm helps you quickly check if the cuff has the right size for the patient. The upper arm should be
between the minimum and maximum scale.
