When: April 28th @ 10AM

How do I sign up a team: Fill out this form with player names, captain, contact
phone number and email. Mail this completed form and the registration fee to
PO Box 33894 Juneau AK, 99801 or drop it off at TMHS attention Julie Herman.

Team Size: All teams are Coed. There are 5 players on the court at a time. There
must be 2 female and 2 male players on the court to start each game. Teams
cannot be larger than 6 players.

How much does it cost: $100 per team received prior to April 20th. After that
date, depending on space available, a late fee of an additional $25.00 will be
charged.

Who is eligible to play: Any person 15 years or older. Players under 18 years
must submit parent permission signature.

The first 5 teams who register will receive free water bottles for each team member.
