NO. DETAILS ACTION ON BY STATUS AS ON 25.08.10
ITC/FABS expressed deep concern on the huge delay in mobilisation
of men and material for the project as a result of which there has
been slow progress at site from the date of issue of LOI (LOI to M/s
L&T was issued on 22nd June 2010).

FABS/ITC once again appraised M/s L&T on the importance on

timelines in this project and informed them that 45 days have
already been lost in the project, M/s L&T would look at various
means and methodologies to make up this delay

FABS/ITC expressed deep concern on the slow and delayed Rock

excavation/ blasting which is delaying the commencement of RCC
works and affecting the overall completion schedule. In order to
overcome this and to facilitate the works at site FABS/ITC informed
that basis the existing levels of rocks encountered at site drawings
would be revised for Plant, GLS, Cooling and RDS building (reduced
depth of excavation from FGL/NGL), as a result the footing and
column design/sizes would undego some changes.However L&T was NOTE
1 advised to submit the action plan for Hard rock Blasting works.

M/s FABS/ITC informed M/s L&T that any delays in the milestones
agreed in the tender/contract would not be entertained and M/s L&T
to look at ways and means to make up the time lost and submit a
catch up schedule for the whole project

M/s L&T informed that 90% of Initial mobilisation and construction of

temporary structure works completed and the team will be
concentrating on Main Civil works to achieve the target within the
agreed contract timeline.

Further L&T agreed to mobilize the following for Excavation and

Filling as,
a)Addtional Excavator
b)Vibromax roller
c)Chiselling
d)Dozer
e)Tanker
M/s FABS/ITC expressed serious concern on M/s L&T not adhering to
the commitments made by their team during the Progress review
meeting held on 16th July 2010.
2 NOTE
The various commitments made by M/s L&T on 16th July 2010 were
discussed in detail and the following was the outcome.

ITC/FABS expressed concern on delay of pouring the first RC concrete

in plant building which has been planned on 22nd July’ 10 and
mobilisation of manpower is not upto the requirement of 433 Nos -
Mason, Carpenter & Fitter + helpers(Available manpower only 113
Nos for main civil works) for the August Month.

L&T informed that the Plant building RC concrete will be commenced 8/10/10 Commenced on 20/08/2010
on 10th August 10

Also confirmed that the mobilisation of manpower will be reach 280

3 Nos by 16th August 2010 and 430 Nos will be reached by 25th August 8/16/10 40 out of 80 nos fitter available.Fitters expected to reach
2010 at 27/08/10

L&T committed to Mobilize the Column Shuttering materials as 8/25/10

follows,
a)Plant Building – 20/08/10 8/20/10 4nos of (1.8m) size+4nos of (3.6m) size ready
b)GLS Building – 10/08/10 8/10/10 4nos of (1.8m) size+4nos of (3.6m) size ready
c)Cooling Block – 20/08/10 8/20/10 4nos of (1.8m) size ready

L&T informed that the finalization of Brought out earth for filling
works schedule would be submitted shortly

Project Specific Quality Plan:

M/s L&T committed to resubmit the complete Project specific Quality 8/14/10
plan by 14th August 2010 against commitment of 19th July 2010. Submitted on 12/08/10

M/s FABS/ITC informed that the submitted Internal and external

testing plan is not as per our project requirement and M/s FABS/ITC
emphasized that no deviations on Quality and safety based on the
contract requirement. L&T

L&T agreed to resubmit the inspection testing plan by 14/08/10. 8/14/10 Submitted on 12/08/10

L&T informed that the checklist & work method statement new
activity would be submitted one week prior to commencement of Record
new activity.

Project Execution Plan:

5 M/s L&T committed to submit the same by 10th August 2010 against 8/10/10 L&T In progress
commitment of 21st July 2010.

Safety:

JSA
M/s L&T informed that JSA would be submitted 1 week or 10
days prior to commencement of any activity. Note

Site Illumination, Access lighting and Task lighting

M/s L&T confirmed that the above requirements would be 8/10/10 Completed except RDS block
arranged by 10th August 2010.

Strengthening of Electrical poles.

M/s L&T informed that the wherever the electrical poles are 8/8/10 Completed except end poles.
not in a vertical position, that pole will be strengthened by
08th August 2010

Safety steward
M/s L&T informed that the required safety stewards (1:50)
 will be deployed by 10th August 2010 for the current 8/10/10 Available nos 4 out of 8nos
manpower strength. Further the 1:50 ratio will be maintained
up to project end.

6 Access Ladder
M/s L&T informed that additional 8 Nos of ladder would be 8/11/10 Mobilized on 18/08/10
mobilised by 11th August 2010 for excavation pit works.

House Keeping team

M/s L&T informed that the colour code system on helmets Note Implemented
will be arranged for Housekeeping team.

Temporary shed @ site

M/s L&T informed that temporary shed would be arranged for 8/10/10 Necessary sheds completed
labours by 10th August 2010.

Cable management system.

M/s FABS/ITC informed that Insulation joint will not be Note In progress
allowed for connecting the cable; it would be only with
Industrial cable connector.

Display board
M/s FABS/ITC informed that a safety display board to be Immediately Completed
displayed for each building with the details of building,
building plan, contact person and emergency contact person.

M/s L&T informed that would be displayed immediately 8/12/10

First aid room

M/s L&T informed that equipped first aid room would be 8/16/10 Ready on 11/08/10
ready by 12th August 2010 against the commitment of 30th

Induction training room

Hard barricading

Meeting & Reports:

Monthly Reports: FABS raised concern on Monthly reports not
being submitted. M/s L&T informed that they would correct 8/12/10 submitted on 12/08/2010

Authorization letter for Project manager : M/s L&T informed 8/20/10 Not submitted

Mixer Machine with weigh batchers/ Mini Batching plant

M/s FABS/ITC informed that mobilised mixer machine with weigh
batcher is not up to our project requirement. 8/20/10 L&T Not Replaced
8 M/s L&T informed that it will be replaced by 20th August 2010.
Further M/s L&T informed Boom placer would be mobilised on 15th 9/15/10 L&T
September 2010.

Quality Lab at site: Lab commissioned,except instruments

core cutter,standard procter density testapparatus,vernier
9 M/s L&T informed the team that the site quality lab would be set-up 8/10/10 calipers.
and running by 10th August against commitment of 5th August 2010.
Batching Plant:
M/s L&T confirmed that Batching Plant would be Commissioned by 8/18/10 Commissioned on 23/08/10
18th August 2010 against the commitment of 05th August 2010.

M/s ITC/FABS advised M/s L&T that the third party approval is
required for Design Mix & L&T agreed for the same & same will be 8/25/10 L&T Submitted on 23/08/10
10 submitted along with 7days trial mix test results by 25th August 2010.

M/s ITC/FABS informed that the 3 days cube test to be done if

procuring the RMC concrete from approved supplier of M/s Ultra Record
tech. Also L&T to deploy their representative during concreting from
RMC plant.
L&T agreed the same.

M/s FABS/ITC once again reminded that minimum 8 Nos of site

engineers with 7 years experienced staff required for this project and 8/10/10 L&T 5 Engineers available at the site.
