INSTRUCTIONS

LIST OF DOCUMENTS REQUIRED TO BE ATTACHED(UPLOAD) WITH THE APPLICATION FORM

1. Evidence of having paid examination fee (Bank challan /Original Bank Draft / Pay Order etc.).

2. Two recent & alike Photographs, one to be attested on the frant side.

3. All attesters must be one of the followings:

☛ Fellow of the College of Physicians & Surgeons Pakistan with his Fellowship Number.
☛ Principal/Professor of PMDC approved Medical College/Postgraduate Medical Institute
Medical Superintendent / Head of the Medical Institutions

Theory examination is conducted at Karachi, Hyderabad, Quetta, M ultan, Lahore, Islamabad, Peshawar,
Kathmandu (Nepal) and Saudia Arabia (Riyadh).

The College can hold examination in any of the listed cities of the country depending on the number of
candidates and logistic facilities available in a city irrespective of the choice given in the Box above.
